目的:探讨签约式健康管理模式对社区心血管病患者病情控制和缓解的效果。方法:以2015年1月至2015年7月期间我社区服务中心自愿参与本研究的214例心血管病患者为研究对象,并随机分为观察组(107例)和对照组(107例),对照组运用常规社区服务模式对患者进行社区医疗服务干预,观察组运用签约式健康管理模式对患者进行社区医疗服务干预,对比分析两组患者相关指标的控制达标率差异。结果:观察组患者干预后TC、TG、LDL-C、HDL-C和血压的控制达标率显著高于对照组,比较均具有显著差异性(P<0.05)。结论:签约式健康管理模式可有效控制社区心血管病患者的TC、TG、LDL-C、HDL-C水平及血压稳定状况,对于社区心血管病患者病情控制和缓解起到一定的效果,值得推广。
Objective: To explore the effect of contractual health management on disease control and relief in community-based cardiovascular patients. Methods: A total of 214 cardiovascular patients voluntarily participated in this study from January 2015 to July 2015 in our community service center and were randomly divided into observation group (107 cases) and control group (107 cases) The control group used routine community service model to intervene patients in community medical service. The observation group used community health service intervention to patients with contracted health management mode, and compared the control compliance rates of the two groups of patients. Results: The rates of TC, TG, LDL-C, HDL-C and blood pressure control in observation group were significantly higher than those in control group (P <0.05). Conclusion: The contracted health management model can effectively control the levels of TC, TG, LDL-C and HDL-C and the steady state of blood pressure in the community cardiovascular patients, and has certain effect on the control and alleviation of the disease in the community cardiovascular patients.
