为探讨肾上腺髓质素 (ADM)在肾小管间质病变中的作用 ,采用放射免疫法测定了 2 6例合并小管间质病变的原发性肾小球肾炎病人血浆ADM水平 ,并在培养的人近曲肾小管上皮细胞系 (HK 2 )观察了ADM对胶原合成和分泌的影响。结果表明 ,小管间质轻度病变组和重度病变组血浆ADM水平较对照组分别增加了 1 2倍和 2 2倍 (P <0 0 1) ,其中间质重度病变组较轻度病变组血浆ADM水平高出 0 5倍 (P <0 0 1) ;ADM对静止的HK 2细胞胶原合成和分泌无明显影响 ,但能够抑制TGF β1刺激的胶原合成和分泌 (P <0 0 1)。提示合并小管间质病变的原发性肾小球肾炎病人血浆ADM水平升高 ,可能为一种代偿性的保护作用。
To investigate the role of adrenomedullin (ADM) in tubulointerstitial lesions, plasma ADM levels in 26 patients with primary glomerulonephritis with tubulointerstitial lesions were measured by radioimmunoassay. Human proximal tubular epithelial cell line (HK 2) observed the effect of ADM on collagen synthesis and secretion. The results showed that the levels of ADM in patients with mild and moderate tubulointerstitial lesions increased by 12-fold and 22-fold respectively (P <0.01), and the levels of plasma ADM (P <0.01). ADM had no effect on the synthesis and secretion of collagen in resting HK2 cells, but inhibited the TGFβ1-stimulated collagen synthesis and secretion (P <0.01). Prompted tubulointerstitial lesions in patients with primary glomerulonephritis plasma ADM levels may be a compensatory protective effect.
