目的探讨中老年骨质疏松与下肢动脉粥样硬化的相关性。方法 110例中老年患者行骨密度及下肢动脉彩超等检查,根据患者骨密度值分为骨量正常组43例,骨量减少组42例,骨质疏松组25例,比较3组收缩压、舒张压、血清白蛋白、血钙、血肌酐、血脂、血尿酸、空腹血糖及动脉粥样硬化积分(Crouse积分)差异;根据Crouse积分将患者分为无动脉粥样硬化组40例,轻度动脉粥样硬化组48例及中重度动脉粥样硬化组22例,比较各组间腰椎骨密度T值及左髋关节骨密度T值差异;分析骨密度T值与Crouse积分相关性。结果骨质疏松组空腹血糖[(6.52±2.97)mmol/L]高于骨量正常组[(5.56±1.11)mmol/L](P<0.05);骨质疏松组Crouse积分[(14.00±9.00)分]高于骨量正常组[(8.00±5.00)分]和骨量减少组[(8.00±7.00)分](P<0.05);不同骨密度组间收缩压、舒张压、血清白蛋白、血钙、血肌酐、血脂、血尿酸比较差异均无统计学意义(P>0.05);中重度动脉粥样硬化组腰椎骨密度T值(-1.90±0.71)低于轻度动脉粥样硬化组(-0.98±0.57)(P<0.05),中重度动脉粥样硬化组左髋关节骨密度T值(-1.28±0.35)低于无动脉粥样硬化组(-0.40±0.17)及轻度动脉粥样硬化组(-0.63±0.22),差异均有统计学意义(P<0.05);骨密度T值与Crouse积分呈负相关(r=-0.218,P=0.022)。结论中老年人群Crouse积分越高,提示骨密度越低。
Objective To investigate the relationship between osteoporosis and lower limb atherosclerosis in middle-aged and elderly people. The difference of T value of lumbar vertebral bone mineral density and left hip bone mineral density between groups was compared. The correlation between T value and Crouse integral was analyzed. Results The fasting blood glucose (6.52 ± 2.97) mmol / L in osteoporosis group was significantly higher than that in normal bone group (5.56 ± 1.11 mmol / L, P <0.05) ) Was higher than those in the normal bone mass group (8.00 ± 5.00) and in the osteopenia group (8.00 ± 7.00) (P <0.05); systolic blood pressure, diastolic blood pressure, serum albumin , Serum calcium, serum creatinine, blood lipid and serum uric acid had no significant difference (P> 0.05). In moderate and severe atherosclerosis group, BMD of lumbar spine (-1.90 ± 0.71) was lower than that of mild atherosclerosis Group (-0.98 ± 0.57) (P <0.05). The T value of left hip joint in moderate-severe atherosclerosis group (-1.28 ± 0.35) was lower than that in non-atherosclerosis group (-0.40 ± 0.17) Atherosclerosis group (-0.63 ± 0.22), the difference was statistically significant (P <0.05); bone mineral density T value was negatively correlated with Crouse score (r = -0.218, P = 0.022). Conclusion The higher the Crouse score in middle-aged and elderly people, the lower the BMD.
