目的了解该院门诊处方情况,提高合理用药水平。方法按照《医院处方点评管理规范(试行)》将不合理处方再分为不规范处方、不适宜处方和超常处方3类,并进行统计分析。结果 2015年1~5月医院门诊药师共调剂处方334 401张,收集干预的不合理处方102张(0.31‰)。其中不规范处方占7.84%(8/102),不适宜处方占91.18%(93/102),超常处方占0.98%(1/102)。结论该院处方存在一定不合理情况,处方点评可以了解临床用药情况,通过发现问题解决问题可以不断提高临床用药水平和药师业务能力,确保患者用药安全、有效。
Objective To understand the outpatient prescriptions in this hospital and improve the rational use of medicine. Methods According to “Management Regulations of Hospital Prescription Reviews (for Trial Implementation)”, the unreasonable prescriptions were further divided into non-standard prescriptions, unsuitable for prescription and supernormal prescriptions, and statistical analysis was conducted. Results From January to May 2015, a total of 334 401 prescriptions were dispensed by outpatient pharmacists in hospitals, and 102 (0.31 ‰) of unreasonable prescriptions were collected. Among them, the non-standard prescriptions accounted for 7.84% (8/102), the unqualified prescriptions accounted for 91.18% (93/102) and the abnormal prescriptions accounted for 0.98% (1/102). Conclusion There are some unreasonable prescriptions in this hospital. The prescription reviews can understand the situation of clinical medication. By finding problems to solve problems, we can continuously improve the level of clinical medication and the ability of pharmacists to ensure the medication safety and effectiveness.
