目的评价2003年至2009年卫生部/全球疫苗免疫联盟合作项目(GAVI项目)在甘肃省榆中县的实施效果。方法 2003年至2009年在甘肃省榆中县按照容量比例概率抽样法每年分别随机抽取210名12～18个月龄儿童和家长,调查乙型肝炎(乙肝)疫苗接种和乙肝防治知识的知晓情况。结果 GAVI项目实施7年来,新生儿乙肝疫苗首针及时接种率从最初的48.6%上升至100.0%,全程接种率从53.3%提高到98.6%,接种儿童家长乙肝防治知识知晓率提高至90.0%。5岁以下儿童乙肝报告发病率降低至0.69/10万。项目实施中期120名儿童HBsAg阳性率1.7%,抗-HBs阳性率为89.2%。结论国际合作项目的实施取得了较理想的效果,大大提高了乙肝疫苗接种率和知晓率。如何巩固和提高乙肝疫苗接种率是当前亟待解决的问题。
Objective To evaluate the implementation effect of the Ministry of Health / GAVI Alliance Project (GAVI Project) in Yuzhong County, Gansu Province from 2003 to 2009. Methods A total of 210 children aged 12-18 months and their parents were randomly selected from Yuzhong County, Gansu Province according to the Probability of Sampling Method of Capacity, from 2003 to 2009 to investigate the knowledge of Hepatitis B (hepatitis B) vaccination and prevention and treatment of hepatitis B . Results In the 7 years since the GAVI project was implemented, the timely first-dose inoculation of newborns with hepatitis B vaccine increased from 48.6% to 100.0% and the overall vaccination rate increased from 53.3% to 98.6%. The awareness rate of hepatitis B prevention and control among vaccinated children increased to 90.0%. The incidence of hepatitis B in children under 5 years of age was reduced to 0.69 / 100,000. In the middle of the project, the HBsAg positive rate of 120 children was 1.7% and the positive rate of anti-HBs was 89.2%. Conclusion The implementation of international cooperation projects has achieved satisfactory results, greatly improving the hepatitis B vaccination rate and awareness rate. How to consolidate and improve the vaccination rate of hepatitis B is the urgent problem to be solved.
