胰腺性脑病是胰腺炎相当常见的并发症,表现为精神错乱、视和听幻觉、行为异常和精神运动性激动。有时可伴有局灶性神经体征和痉挛危象,最常见于夜间。作者研究了17例急性胰腺炎患者,无嗜酒者。无难以控制的水电解质紊乱,均为70岁以下。全部病例均复查血淀粉酶、尿淀粉酶和血脂酶,每日查电解质、血糖、血钙、氧分压、二氧化碳分压、碳酸氢盐和氢离子浓度。在胰腺炎开始时及10天后查脑脊液细胞、蛋白、淀粉酶、脂酶、脂类和胆固醇。用一组19例急性脑血管病患者和22例神经症的病人做对照,以观察脑脊液参数的正常范围。血和脑脊液中的脂酶按Bang氏法测定。以超过对照组平均值2个标准差为异常。结果:17例患者脑脊液的蛋白、淀粉酶、胆固醇和脂类测定均正常。11例脑脊液脂酶测定正常者无
Pancreatic encephalopathy is a fairly common complication of pancreatitis characterized by insanity, visual and auditory hallucinations, abnormal behavior and psychomotor agitation. Sometimes accompanied by focal neurological signs and spasm crisis, the most common at night. The authors studied 17 patients with acute pancreatitis, no alcoholics. No difficult to control the water and electrolyte disorders, are under 70 years of age. Blood amylase, urinary amylase and lipase were reviewed in all cases. Electrolytes, blood glucose, serum calcium, partial pressure of oxygen, partial pressure of carbon dioxide, bicarbonate and hydrogen ion concentrations were examined daily. Cerebrospinal fluid cells, proteins, amylases, lipases, lipids and cholesterol were examined at the beginning of pancreatitis and after 10 days. A group of 19 patients with acute cerebrovascular disease and 22 patients with neurosis were compared to observe the normal range of cerebrospinal fluid parameters. Lipase in blood and cerebrospinal fluid was determined by the Bang’s method. To exceed the average of two standard deviations of the control group is abnormal.
