目的:探讨Smac和Survivin蛋白在直肠癌组织中的表达及临床意义。方法:应用免疫组织化学技术检测Smac和Survivin蛋白在60例直肠癌组织、30例直肠腺瘤组织及10例癌旁正常黏膜组织中的表达。结果:Smac在直肠癌组织、直肠腺瘤及癌旁正常组织的表达率分别为71.67%(43/60)、96.67%(29/30)和100.00%(10/10),差异有统计学意义,χ2=13.981,P=0.001。Sur-vivin在直肠癌组织、直肠腺瘤及癌旁正常组织的表达率分别为90.00%(54/60)、80.00%(24/30)和20.00%(2/10),差异有统计学意义,χ2=9.539,P=0.002。Smac和Survivin蛋白的表达水平与患者的年龄、性别、肿瘤部位、术前CEA水平、肿瘤分化程度和远处转移无关(P值均>0.05),但与TNM分期和淋巴结转移有关,P值均<0.05。Smac阳性率与淋巴结转移呈负相关,r=-0.079,P<0.05;Survivin阳性率与淋巴结转移呈正相关,r=0.236,P<0.05。Smac和Survivin在直肠癌中的表达呈负相关,r=-0.086,P<0.05。结论:Smac和Survivin蛋白表达与直肠癌的病理分期和淋巴结转移密切相关,Smac在直肠癌中低表达,其表达与Survivin呈负相关,两者在直肠癌细胞凋亡信号传导网络中可能发挥重要的作用。中华肿瘤防治杂志,2011,18(12):956-960
Objective: To investigate the expression and clinical significance of Smac and Survivin protein in rectal cancer. Methods: Immunohistochemical technique was used to detect the expression of Smac and Survivin protein in 60 cases of rectal cancer, 30 cases of rectal adenoma and 10 cases of adjacent normal mucosa. Results: The positive rates of Smac in rectal cancer tissues, rectal adenomas and adjacent normal tissues were 71.67% (43/60), 96.67% (29/30) and 100.00% (10/10), respectively, with significant difference , χ2 = 13.981, P = 0.001. The positive rates of Sur-vivin in rectal cancer tissues, rectum adenomas and adjacent normal tissues were 90.00% (54/60), 80.00% (24/30) and 20.00% (2/10), respectively, with significant difference , χ2 = 9.539, P = 0.002. The expression levels of Smac and Survivin protein were not related with age, gender, tumor location, preoperative CEA level, tumor differentiation and distant metastasis (all P values> 0.05), but were correlated with TNM stage and lymph node metastasis, P values <0.05. The positive rate of Smac was negatively correlated with lymph node metastasis (r = -0.079, P <0.05). There was a positive correlation between the positive rate of Survivin and lymph node metastasis (r = 0.236, P <0.05). The expression of Smac and Survivin in rectal cancer was negatively correlated, r = -0.086, P <0.05. CONCLUSIONS: Smac and Survivin protein expressions are closely related to the pathological stage and lymph node metastasis of rectal cancer. Smac is low expressed in rectal cancer and its expression is negatively correlated with Survivin, both of which may play an important role in the signal transduction network of rectal cancer Role.
