目的探讨和建立有效降低脑卒中发病、死亡及危险因素水平的长效机制与资源管理模式。方法建立以区卫生局领衔,社区人群脑卒中防治体系,医院与全社区卫生服务中心建立双向转诊制度,2级医院医师进入社区讲解脑血管病预防知识,训练社区医生防治卒中水平,地段医院与全社区卫生服务中心建立双向转诊制度,在社区人群中开展危险因素水平的调查与评估,提高社区居民防治脑卒中知识的知晓率及社区卫生对脑卒中的保健率。结果社区医生对脑卒中防治知识的合格率从培训前的21%提高到培训后的98%,社区居民防治脑卒中知识的知晓率明显提高,社区卫生对脑卒中的保健率逐年提高,脑内血肿穿刺引流及溶栓病例显著增加,近4年长辛店地区脑卒中患病率和病死率逐年下降。结论利用地段医院自愿与社区卫生整合防治脑卒中的方法是值得广泛推广的防治脑卒中的最佳模式。
Objective To explore and establish a long-term mechanism and resource management model that can effectively reduce the incidence of stroke, death and risk factors. Methods To establish a two-way referral system with the leadership of the District Health Bureau and community-based stroke prevention and control system. The hospital and the community-wide health service center established a two-way referral system. Level-2 hospital doctors entered the community to explain prevention knowledge about cerebrovascular disease and trained community doctors to prevent and treat stroke. Establish a two-way referral system with the community health service center to carry out investigation and assessment of the risk factors in the community population so as to raise the awareness rate of community residents in stroke prevention and control and the community health care rate of stroke. Results The prevalence rate of community doctor on stroke prevention and treatment was increased from 21% before training to 98% after training. The awareness rate of community residents in stroke prevention and control was significantly increased. The rate of community health care for stroke increased year by year, Hematoma puncture and drainage and thrombolytic cases increased significantly, nearly 4 years Changxindian area stroke prevalence and mortality decreased year by year. Conclusions The method of using hospital voluntarily and integrating community health to prevent and treat stroke is a best way to prevent and cure stroke.
