目的探讨产KPC-2酶肺炎克雷伯菌毒力、血清型及基因分型,为临床诊治肺炎克雷伯菌感染提供参考依据。方法收集2015年1月-2016年1月在医院住院243例患者各种标本中培养分离出肺炎克雷伯菌菌株147株,采用PCR扩增技术对allS、rmpA、kfuBC、mrkD、cf29a、uge、fimH、wabG、ureA等9种毒力基因及K1、K2、K5、K20、K54、K57等6种血清型进行检测,分析结果。结果 52株碳青霉烯类敏感,均不产KPC-2酶,95株耐药,均产KPC-2酶,构成比为35.37%、64.63%;不产KPC-2酶的菌株allS基因阳性率高于产KPC-2酶菌株(P<0.05)。结论产KPC-2酶肺炎克雷伯菌毒力allS基因较不产KPC-2酶肺炎克雷伯菌下降,血清型与不产KPC-2酶肺炎克雷伯菌无明显差异。
Objective To investigate the virulence, serotype and genotyping of Klebsiella pneumoniae producing KPC-2 enzyme and to provide a reference for clinical diagnosis and treatment of Klebsiella pneumoniae infection. Methods A total of 147 strains of Klebsiella pneumoniae were isolated from 243 specimens of hospitalized patients from January 2015 to January 2016 in our hospital. PCR was used to screen allS rmpA kfuBC mrkD cf29a uge , FimH, wabG, ureA and other 9 kinds of virulence genes and K1, K2, K5, K20, K54, K57 and other 6 kinds of serotypes were detected and analyzed. Results 52 strains were susceptible to carbapenem. No KPC-2 enzyme and 95 strains were resistant to KPC-2 enzyme. The KPC-2 enzymes accounted for 35.37% and 64.63%, respectively. The rate of producing KPC-2 enzyme was higher (P <0.05). Conclusion KPC-2 enzyme producing Klebsiella pneumoniae virulence allS gene KPC-2 enzyme-producing Klebsiella pneumoniae less, serotypes and KPC-2-producing Klebsiella pneumoniae no significant difference.
