目的探讨尿N-乙酰-β-氨基葡萄糖苷酶(NAG)联合尿微量蛋白检测在糖尿病和高血压病早期肾损伤的诊断及临床价值。方法收集152例糖尿病和122例高血压病患者及60例正常对照者,对其尿β2微球蛋白(β2-MG)、免疫球蛋白G(IgG)、微量白蛋白(mALB)、α1微球蛋白(α1-MG)及NAG水平进行检测并分析。结果糖尿病与高血压病尿蛋白阳性组4种微量蛋白及NAG水平明显高于尿蛋白阴性组与对照组(P<0.001),尿蛋白阴性组4种微量蛋白及NAG水平高于对照组(P<0.05)。糖尿病与高血压病尿蛋白阴性性组NAG联合4种尿微量蛋白检测阳性率明显高于各单项检测阳性率(P<0.001),与4种微量蛋白联合检测阳性率比较亦升高(P<0.05)。结论尿N-乙酰-β-氨基葡萄糖苷酶(NAG)与尿微量蛋白联合检测对早期肾脏损害的诊断具有重要临床价值。
Objective To investigate the diagnostic value and clinical value of urinary N-acetyl-β-glucosaminidase (NAG) combined with urinary microalbuminuria in early stage of diabetic nephropathy and hypertension. Methods 152 patients with diabetes mellitus and 122 patients with hypertension and 60 healthy controls were enrolled in this study. Their urinary β2-MG, IgG, mALB, α1 microspheres Protein (α1-MG) and NAG levels were detected and analyzed. Results The levels of four trace proteins and NAG in urine and urine protein positive group were significantly higher than those in urine negative group and control group (P <0.001). The levels of four trace proteins and NAG in urine protein negative group were significantly higher than those in control group <0.05). The positive rate of 4 urinary microalbuminuria patients with diabetes mellitus and hypertension with urinary protein negative group was significantly higher than that of each individual test (P <0.001), and positive rate with 4 microalbumin (P < 0.05). Conclusion The combination of urine N-acetyl-β-glucosaminidase (NAG) and urinary microalbumin has important clinical value in the diagnosis of early renal damage.
