目的探讨血压正常高值者中心动脉压及其反射波与动脉弹性的关系。方法选择正常血压者159例,血压正常高值者149例,高血压者153例,应用大动脉测量仪测量中心动脉压及其反射波;应用脉搏波传导速度测定仪测定颈桡动脉脉搏波传导速度(crPWV)。结果血压正常高值组中心动脉收缩压(CSP)、舒张压(CDP)、中心脉压(CPP)、平均收缩压(CMSP)、平均舒张压(CMDP)、收缩末压(CESP)、第一峰值血压与最小血压之差(P1Height)及crPWV值均高于正常血压组,低于高血压组(P<0.01或P<0.05)。多元线性回归分析显示,CDP、CMDP、CPP是crPWV的影响因素(β值分别为0.314,0.223,0.196;均P<0.05)。结论血压正常高值者中心动脉压及其反射波已经增高,动脉弹性已经减退,CDP和CPP是影响动脉弹性的主要因素。
Objective To investigate the relationship between central arterial pressure and its reflected wave and arterial elasticity in patients with normal blood pressure. Methods 159 cases of normotensive blood pressure, 149 cases of normal high blood pressure, hypertension in 153 cases, the application of aortic measurement of central arterial pressure and its reflected wave; pulse wave velocity meter used to measure the radial artery pulse wave velocity (crPWV). Results CSP, CDP, CPP, CMSP, CESP in the high normotensive group were higher than those in the first normotensive group The difference between peak blood pressure and minimum blood pressure (P1Height) and crPWV values were higher than those in normotensive group and lower than those in hypertensive group (P <0.01 or P <0.05). Multivariate linear regression analysis showed that CDP, CMDP and CPP were the influencing factors of crPWV (β values were 0.314,0.223,0.196; all P <0.05). Conclusion The patients with high normotension have higher central arterial pressure and their reflex waves. Arterial elasticity has decreased. CDP and CPP are the main factors affecting arterial elasticity.
