目的调查塑料垃圾回收工人的甲状腺激素类指标及其危险因素,旨在评价增塑剂邻苯二甲酸二(2-乙基)己酯[(Di(2-ethylhexyl)phthalate,DEHP]对职业人群甲状腺功能的影响,补充流行病学证据。方法 2012年11月以淮安市某塑料垃圾回收工业区为暴露区,该暴露区相邻县区的农业区作为对照区,从中各选取72名塑料垃圾回收工人和64名性别和年龄匹配的农民作为研究对象。采用调查表问卷方式收集研究对象的个人信息。用试剂盒检测血清中促甲状腺激素(TSH)、三碘甲状腺素原氨酸(T3)和甲状腺素(T4)水平,计算T3/T4比值。用logistic回归模型分析以上标志物的影响因素。结果暴露组和对照组人群的血清中TSH水平差异无统计学意义(P>0.05)。相比对照组,暴露组血清中T3(中位数:1.04 ng/ml;四分位间距:0.79~1.21 ng/ml)和T3/T4比值(中位数:1.42;四分位间距:1.12~1.73)较高(P<0.01),但血清中T4水平(中位数:76.02 ng/ml;四分位间距:62.64~83.57 ng/ml)却明显下降(P<0.01)。logistic回归模型显示,塑料垃圾回收职业史是血清中T3(OR:1.908;95%CI:1.450~2.635)、T4水平(OR:1.367;95%CI:1.412~1.701)和T3/T4比值(OR:3.457;95%CI:2.129~5.079)的唯一影响因素。结论该地塑料垃圾回收工人甲状腺功能处于代偿应激状态,这可能与其职业暴露于高浓度DEHP有关。
Objective To investigate the indexes of thyroid hormones and their risk factors in plastics refuse collection workers to evaluate the effects of plasticizer di (2-ethylhexyl) phthalate (DEHP) on occupational groups Thyroid function and supplement the epidemiological evidence.Methods In November 2012, a plastic garbage recycling industrial area in Huaian City was exposed and the agricultural area in the adjacent counties in the exposed area was taken as control area, 72 plastic garbage Recycling workers and 64 peasants who were sexually and age-matched were used as research subjects.The questionnaire was used to collect the personal information of the subjects.Serum thyroid stimulating hormone (TSH), triiodothyronine (T3) And thyroxine (T4) levels were calculated and the ratio of T3 / T4 was calculated.The influencing factors of the above markers were analyzed by logistic regression model.Results There was no significant difference in serum TSH levels between the exposed group and the control group (P> 0.05) Compared with the control group, T3 (median: 1.04 ng / ml; interquartile range: 0.79-1.21 ng / ml) and T3 / T4 ratio in the exposed group (median: 1.42; 1.73) (P <0.01), but serum T4 level (median : 76.02 ng / ml; interquartile range: 62.64-83.57 ng / ml) was significantly decreased (P <0.01) .Logistic regression model showed that the occupational history of plastic waste recycling was T3 in serum (OR: 1.908; 95% CI: (OR: 1.367; 95% CI: 1.412-1.701) and T3 / T4 ratio (OR: 3.457; 95% CI: 2.129-5.079) .Conclusion The plastic waste recycling workers Thyroid function is under compensatory stress, which may be related to occupational exposure to high concentrations of DEHP.
