目的探讨原发性高血压(EH)患者血浆脑利钠肽(BNP)含量及其与左心室肥厚(LVH)、左心室舒张功能(LVDF)的关系。方法采用免疫放射分析法测定122例EH患者和36例对照组血浆BNP浓度;用心脏彩色多普勒超声诊断仪器测定EH患者左室质量指数(LVMI)、左室射血分数(LVEF)和E/A比值。对BNP与LVMI、E/A比值作直线相关分析。结果 EH患者BNP浓度显著高于对照组(P<0.01),且EH伴LVH者BNP浓度明显高于EH无LVH者,差异有统计学意义(P<0.01);EH伴LVH者血浆BNP浓度与LVMI呈显著正相关(r=0.45,P<0.01),与E/A比值呈显著负相关(r=-0.68,P<0.01)。结论 EH患者血浆BNP水平明显升高,且与LVH、LVDF密切相关;BNP浓度的测定可望成为EH患者LVH及LVDF障碍的一项诊断指标。
Objective To investigate the relationship between plasma brain natriuretic peptide (BNP) and left ventricular hypertrophy (LVH) and left ventricular diastolic function (LVDF) in patients with essential hypertension (EH). Methods Plasma BNP levels were measured in 122 patients with EH and 36 controls by immuno-radiometric analysis. Left ventricular mass index (LVMI), left ventricular ejection fraction (LVEF) and E / A ratio. Linear correlation analysis between BNP and LVMI, E / A ratio. Results The plasma BNP levels in patients with EH were significantly higher than those in controls (P <0.01), and the BNP levels in EH with LVH were significantly higher than those without EH in LVH (P <0.01) LVMI was positively correlated (r = 0.45, P <0.01), and negatively correlated with E / A ratio (r = -0.68, P <0.01). Conclusions Plasma BNP levels in patients with EH are significantly increased, and are closely related to LVH and LVDF. The determination of BNP concentration is expected to be a diagnostic indicator of LVH and LVDF disorders in patients with EH.
