目的:探讨组蛋白H3乙酰化状态与宫颈HPV感染的相关性,及其分别与CINⅠ级转归的关系。方法:(1)选择2006年1月~2008年1月我院治疗的正常宫颈、CINⅠ、CINⅡ~Ⅲ、宫颈癌组织各30例,用免疫组化技术检测其组蛋白H3乙酰化表达水平;(2)对2006年1月~2008年1月仁济医院阴道镜下活检确诊为CINⅠ级的161例患者,用免疫组化技术检测组织中组蛋白H3乙酰化表达的水平,采用凯普技术检测宫颈HPV感染分型。结果:(1)CIN病变组组蛋白H3乙酰化水平与正常宫颈组和宫颈癌组的差异有统计学意义(P<0.01)。CINⅠ和CINⅡ~Ⅲ组差异有统计学意义(P<0.05);(2)组蛋白H3乙酰化表达水平增加,进展为CINⅡ级以上的发生率进行性升高,(+++)组最高达80%,差异有统计学意义(P<0.05);(3)宫颈高危型HPV16/18感染的CINⅠ级,其组蛋白H3乙酰化表达水平明显高于低危型和非16/18高危型(P<0.05);进展为CINⅡ级以上发生率与低危型和非16/18高危型的差异有统计学意义(P<0.05);(4)不同的治疗方案对CINI级转归的影响不同,LEEP术后随访6月无1例复发,HPV转阴率77.94%(P<0.05)。结论:组蛋白H3乙酰化水平和HPV感染类型与CINI级转归密切相关。
Objective: To investigate the correlation between histone H3 acetylation status and cervical HPV infection and its relationship with CIN Ⅰ-level outcome. Methods: (1) 30 cases of normal cervix, CINⅠ, CINⅡ ~ Ⅲ and cervical cancer tissues treated in our hospital from January 2006 to January 2008 were selected. The expression of histone H3 was detected by immunohistochemistry. (2) From 161 January 2006 to January 2008 161 patients with clinically diagnosed as CINⅠin the colorectal biopsy of Renji Hospital were examined for the level of histone H3 acetylation by immunohistochemistry, Cervical HPV infection detection typing. Results: (1) The histone H3 acetylation level in CIN group was significantly different from that in normal cervical group and cervical cancer group (P <0.01). There was significant difference between CINⅠ and CINⅡ ~ Ⅲgroup (P <0.05); (2) The expression of histone H3 acetylation increased, the progress was more than CINⅡ, (P <0.05). (3) The level of acetylation of histone H3 in CINⅠlevel of HPV16 / 18 infection was significantly higher than that in low-risk type and non-high-risk type P <0.05). The difference was statistically significant (P <0.05) between the incidence of CIN level II and above and the risk of non-16/18 high risk type. (4) The effect of different treatment regimens on CINI-grade outcomes was different There was no recurrence after LEEP in June, and the negative rate of HPV was 77.94% (P <0.05). Conclusion: The level of histone H3 acetylation and HPV infection are closely related to CINI grade.
