目的 探讨外周血角蛋白 2 0 (CK 2 0 )基因表达在膀胱移行细胞癌微转移情况及其临床意义。方法 以逆转录多聚酶链反应 (RT PCR)检测 91例膀胱癌不同临床分期及病理分级CK 2 0mRNA外周血表达情况。结果 91例膀胱癌外周血CK 2 0基因表达阳性 47例 (5 1.6% ) ,外周血肿瘤细胞微转移与膀胱癌患者的临床分期密切相关 (P <0 .0 5 ) ,而与病理分级无明显相关(P >0 .0 5 ) ,随访中未发现淋巴结转移的T1~ 2 期患者 ,术后 8个月 2例复发且均为CK 2 0RNA表达阳性。结论 RT PCR方法检测CK 2 0mRNA在外周血表达是判断膀胱癌微转移的重要标志 ,对准确分期及治疗有重要意义。
Objective To investigate the micrometastasis of cytokeratin 20 (CK 2 0) gene in bladder transitional cell carcinoma and its clinical significance. Methods Reverse transcription polymerase chain reaction (RT-PCR) was used to detect the expression of CK 2 0 mRNA in 91 cases of bladder cancer with different clinical stages and pathological grade. Results The positive expression of CK 2 0 in 91 cases of bladder cancer was positive (51.6%). The micrometastasis in peripheral blood was closely related to the clinical stage of bladder cancer (P <0.05), but not to the pathological grade (P0.05) .There were no patients with T1 ~ 2 lymph node metastases at follow-up. There were 2 cases recurrence at 8 months after operation and both of them were positive for CK 2 0RNA. Conclusion The detection of CK20 mRNA in peripheral blood by RT-PCR is an important marker for judging the micrometastasis of bladder cancer. It has important significance for accurate staging and treatment.
