目的:探讨可摘式牙周夹板修复联合米诺环素对重度牙周病患者牙周指标及牙齿美观度的影响。方法:选择中国医科大学附属口腔医院2018年11月至2020年4月收治的重度牙周病患者102例为研究对象,采用随机数字表法分为研究组与对照组,每组51例。常规干预基础上对照组采取可摘式牙周夹板修复,研究组在对照组基础上联用米诺环素。统计两组临床疗效、治疗前后牙周状况[龈沟出血指数(SBI)、菌斑指数(PLI)、牙周袋深度(PD)]、龈沟液炎性因子[转化生长因子β(TGF-β)、单核细胞趋化蛋白1(MCP-1)、白细胞介素6(IL-6)、基质金属蛋白酶8(MMP-8)]及骨代谢指标[骨钙素(BGP)、Ⅰ型前胶原N端前肽(PINP)、Ⅰ型胶原C端肽(CTX)]水平、舒适度及美观度评分、治疗满意度。结果:研究组总有效率[94.12%(48/51)]高于对照组[80.39%(41/51)](χn 2=4.32,n P < 0.05)。治疗后1个月、3个月研究组SBI(1.32±0.41、1.11±0.36)、PLI(1.51±0.44、1.32±0.51)、PD[(3.29±0.70)mm、(2.51±0.63)mm]低于对照组[(1.65±0.39、1.45±0.38、1.92±0.42、1.88±0.49、(5.05±0.79)mm、(3.82±0.86)mm]( n t=4.16、4.63、4.81、5.65、11.90、8.77,均n P < 0.001)。治疗后1个月、3个月研究组龈沟液TGF-β均高于对照组,MMP-8水平均低于对照组(均 n P < 0.001)。治疗后1个月、3个月研究组龈沟液BGP均高于对照组,CTX均低于对照组( n t=-9.97、-10.71、-5.77、-7.40、7.24、16.11,均n P < 0.001)。治疗后1个月、3个月研究组舒适度评分、美观度评分均高于对照组( n t=7.49、8.26、7.84、9.10,均n P < 0.001)。研究组治疗满意度为94.12%,高于对照组的80.39%(χ n 2=4.32,均n P < 0.05)。n 结论:可摘式牙周夹板修复联合米诺环素通过控制牙周炎症,调节骨代谢,有助于增强治疗效果,改善牙周状况,提高重度牙周病患者牙齿舒适度、美观度及治疗满意度。“,”Objective:To investigate the effects of removable periodontal splint combined with minocycline on periodontal indexes and tooth aesthetics in patients with severe periodontal disease.Methods:A total of 102 patients with severe periodontal disease treated in the School and Hospital of Stomatology, China Medical University from November 2018 to April 2020 were included in this study. They were randomly allocated into study and control groups (n n = 51/group). The control group was subject to repair with removable periodontal splint based on routine interventions. The study group was subject to medication with minocycline in addition to the treatments used in the control group. Clinical efficacy, periodontal status (sulcus bleeding index, plaque index, periodontal pocket depth) and gingival crevicular fluid inflammatory factors (transforming growth factor β, monocyte chemoattractant protein-1, interleukin-6, matrix metalloproteinase-8) and bone metabolism indexes [osteocalcin, N-terminal procollagen of type I (PINP), N-terminal propeptide of type I procollagen (PINP), C-terminal telopeptide of type I collagen (CTX) levels], comfort and aesthetics scores, and patient satisfaction were compared between the two groups.n Results:Total response rate was significantly higher in the study group than in the control group [94.12% (48/51) n vs. 80.39% (41/51), n χ2 = 4.32, n P < 0.05]. At 1 and 3 months after treatment, sulcus bleeding index (1.32 ± 0.41, 1.11 ± 0.36), plaque index (1.51 ± 0.44, 1.32 ± 0.51), periodontal pocket depth [(3.29 ± 0.70) mm, (2.51 ± 0.63) mm] were significantly lower in the study group than in the control group [1.65 ± 0.39, 1.45 ± 0.38, 1.92 ± 0.42, 1.88 ± 0.49, (5.05 ± 0.79) mm, (3.82 ± 0.86) mm, n t = 4.16, 4.63, 4.81, 5.65, 11.90, 8.77, all n P < 0.001]. At 1 and 3 months after treatment, the level of transforming growth factor β in the gingival crevicular fluid was significantly higher, and the level of matrix metalloproteinase-8 in the gingival crevicular fluid was significantly lower, in the study group compared with the control group (both n P < 0.001). At 1 and 3 months after treatment, the level of osteocalcin in the gingival crevicular fluid was significantly higher, and the level of C-terminal telopeptide of type I collagen in the gingival crevicular fluid was significantly lower, in the study group compared with the control group ( n t = -9.97, -10.71, -5.77, -7.40, 7.24, 16.11, all n P < 0.001). At 1 and 3 months after treatment, the scores of comfort and aesthetics in the study group were significantly higher than those in the control group ( n t = 7.49, 8.26, 7.84, 9.10, all n P < 0.001). Patient satisfaction in the study group was significantly higher than that in the control group (94.12% n vs. 80.39%, n χ2 = 4.32, n P < 0.05).n Conclusion:Repair with a removable periodontal splint combined with minocycline can increase the therapeutic effects through reducing periodontal inflammation and regulating bone metabolism, thereby improving the periodontal condition, and improving tooth comfort and aesthetics and patient satisfaction in patients with severe periodontal disease.
