目的 :探讨荧光酶联免疫吸附法检测变应性鼻炎特异性抗体的临床诊断意义。方法 :应用UniCAP 10 0E全自动变应原检测仪 ,荧光酶联免疫吸附法检测 84例变应性鼻炎患者 (实验组 )及 2 0名健康人 (对照组 )血清特异性免疫球蛋白E (specificimmunoglobulinE ,SIgE)与血清总免疫球蛋白E(totalimmunoglobulinE ,TIgE)水平 ,并与变应原皮肤试验结果进行比较。结果 :①实验组的变应原螨类(屋尘螨、粉尘螨 )SIgE阳性检出率高达 4 5 % ,屋尘、混合性食物 (虾、蟹、牛奶 )、多价霉菌、蟑螂SIgE分别为 37%、 2 2 %、 10 %、 16 % ;均高于对照组的 5 %、 0、 5 %、 0、 0 (均为P <0 0 1) ;②实验组的TIgE阳性率为 75 % (6 3/ 84 ) ;③实验组上述 5类常见变应原的血清SIgE与皮肤试验的符合率分别为5 1%~ 71% ,与TIgE的符合率则为 38%~ 6 6 %。结论 :荧光酶联免疫吸附法检测变应性鼻炎特异性抗体 ,指标客观、结果准确 ,可作为变应性鼻炎病因诊断的重要依据。
Objective: To investigate the clinical significance of fluorescent enzyme-linked immunosorbent assay in the detection of allergic rhinitis-specific antibodies. Methods: UniCAP 10 0E automatic allergen detector was used to detect the serum specific immunoglobulin E (n = 84) in 84 patients with allergic rhinitis (experimental group) and 20 healthy persons (control group) by fluorescence enzyme-linked immunosorbent assay specificimmunoglobulinE, SIgE) and total serum immunoglobulin E (TIgE) levels, and compared with the allergen skin test results. Results: ① The SIgE positive rate of allergen mites (house dust mite and dust mite) in experimental group was as high as 45%, and house dust, mixed food (shrimp, crab, milk), polymyxin, SIgE (P <0.01). The positive rates of TIgE in the experimental group were 75%, 37%, 22%, 10% and 16% % (6 3/84), respectively; ③ The coincidence rates of serum SIgE and skin test of the above five kinds of common allergens in the experimental group were 51% ~ 71% and 38% ~ 66% with TIgE respectively. Conclusion: Fluorescent enzyme-linked immunosorbent assay for the detection of allergic rhinitis-specific antibodies, objective, accurate results, can be used as an important basis for diagnosis of allergic rhinitis.
