目的探讨食管癌术后不同营养方式对患者肝功能的影响。方法 2012年2月至2014年12月期间在泸州医学院附属医院胸心外科行食管癌(或贲门癌)切除术的患者共160例,根据不同医疗组将患者分成肠内营养(EN)组和全胃肠外营养(TPN)组,每组80例;分析两组患者术后第2 d和术后第7 d血清白蛋白(ALB)、血清前白蛋白(PA)、丙氨酸转氨酶(ALT)、总胆红素(TB)水平的差异。结果术后第2 d,两组患者均有部分患者出现ALB、PA、ALT和TB异常,但差异无统计学意义(P>0.05);术后第7 d,两组患者肝功能指标较术后第2 d均有所改善,但EN组各项肝功能指标显著优于TPN组(P<0.05)。结论与全胃肠外营养相比较,肠内营养更利于食管癌术后肝脏的蛋白合成及术后肝功能的恢复。
Objective To investigate the effect of different nutritional methods on liver function in patients with esophageal cancer after operation. Methods A total of 160 patients undergoing esophagectomy (or cardia cancer) resection in the Department of Cardiac Surgery, Luzhou Medical College Affiliated Hospital from February 2012 to December 2014 were divided into three groups according to different medical groups: enteral nutrition group And total parenteral nutrition (TPN) group with 80 cases in each group. Serum albumin (ALB), serum prealbumin (PA), alanine aminotransferase (ALT), total bilirubin (TB) levels. Results On the 2nd day after operation, some patients in both groups had abnormal ALB, PA, ALT and TB, but the difference was not statistically significant (P> 0.05). On the 7th day after operation, the liver function indexes However, the indexes of liver function in EN group were significantly better than those in TPN group (P <0.05). Conclusion Compared with total parenteral nutrition, enteral nutrition is more beneficial to the postoperative liver protein synthesis and postoperative recovery of liver function in esophageal cancer patients.
