目的初步探讨经胃镜植入化疗粒子行组织内化疗治疗食管癌的近期疗效与不良反应。方法对12例进展期食管癌伴不全梗阻患者于内镜下行食管癌梗阻部位及浸润处多点或密集型植入5-氟尿嘧啶(5-FU)粒子。首先通过消化道造影测出肿瘤区域体积和食管狭窄的长度,计算出需要植入粒子的数量。通过胃镜活检孔插入5-FU粒子专用注射针,直接刺入肿瘤组织内,将5-FU粒子植入。观察近期疗效和不良反应。结果植入术均安全完成,未发生大出血、穿孔等并发症;肝肾功能及血常规检查未见异常。12例中显效3例,有效7例,无效2例,总有效率83.3%。结论植入缓释5-FU对缓解食管癌浸润引起的梗阻有一定作用,副作用小,是一种安全有效的新方法。
Objective To investigate the short-term curative effect and side effects of intragastric chemotherapy in esophageal cancer treated by endoscopic implantation of chemotherapy particles. Methods A total of 12 patients with advanced esophageal carcinoma with incomplete obstruction were implanted with 5-fluorouracil (5-FU) particles in endoscopic esophageal cancer obstruction and infiltration sites. First, the size of tumor area and the length of esophageal stenosis were measured by digestive angiography, and the number of implanted particles was calculated. 5-FU particle-specific injection needle was inserted through the endoscopic biopsy hole, directly penetrated into the tumor tissue, and 5-FU particles were implanted. Observation of short-term efficacy and adverse reactions. Results All the implants were completed safely without complications such as hemorrhage and perforation. There was no abnormal liver and kidney function and blood tests. Of the 12 cases, 3 were markedly effective, 7 were effective, 2 were ineffective and the total effective rate was 83.3%. Conclusion The slow release of 5-FU is a safe and effective method to relieve the obstruction caused by esophageal cancer infiltration.
