目的 探讨血钠浓度的改变对病毒性脑炎患儿预后的影响。方法对1998年7月-2004年3月收治本院的86例病毒性脑炎患儿进行临床分析,所有患儿均在入院后6h内测血清电解质,1-3d内进行脑电图和头颅CT或MRI检查。结果 入院第一天发生电解质紊乱33.72%,其中低钠血症23.26%;重症组低钠血症发生率高于轻症组,且二组差异有显著性;脑电图重度异常和头颅CT或MRI有脑实质受损者,其低钠血症发生率高于无改变者。结论 血钠浓度的改变往往提示病情相对较重,及早发现和治疗低钠血症或高钠血症是改善病毒性脑炎患儿预后的途径之一。
Objective To investigate the effect of serum sodium concentration on the prognosis of children with viral encephalitis. Methods Clinical data of 86 children with viral encephalitis admitted to our hospital from July 1998 to March 2004 were enrolled in this study. All children were tested for serum electrolytes within 6 hours after admission, and EEG and cranial CT or MRI examination. Results On the first day of hospital admission, electrolyte imbalance occurred in 33.72%, of which hyponatremia was 23.26%. The incidence of hyponatremia in severe group was higher than that in mild group, and there was significant difference between the two groups. EEG abnormalities and CT or MRI patients with impaired brain, the incidence of hyponatremia than those without change. Conclusions The change of serum sodium concentration often indicates relatively serious condition. Early detection and treatment of hyponatremia or hypernatremia is one of the ways to improve the prognosis of children with viral encephalitis.
