目的:评价平台转换技术在牙列缺损种植修复中的临床效果。方法:38例牙列缺损种植患者随机分为实验组与对照组,共植入51枚直径为4.5 mm XIVE种植体。实验组:19例27枚种植体选用愈合基台直径为3.8 mm平台转换连接式,重建种植区的结合上皮,3.8 mm修复基台,高含金量贵金属烤瓷冠修复。对照组:19例24枚种植体选用愈合基台、修复基台直径与种植体直径相同的平齐对接连接式,高含金量贵金属烤瓷冠修复。分别于种植体负载前及负载后3、6、12月时行X线检查,测量种植体颈部边缘骨吸收量及软组织健康状况并进行评价。结果:平台转换种植体颈部骨吸收量明显小于对连接种植体(P﹤0.01),两者软组织健康状况差异不明显(P﹥0.05)。结论:与平齐式对接相比,功能负载1年内,临床平台转换设计可保留种植体颈部边缘骨高度。
OBJECTIVE: To evaluate the clinical effect of platform conversion in the repair of dentition defect. Methods: Thirty-eight patients with dental implants were randomly divided into experimental group and control group, and 51 implants of 4.5 mm in diameter were implanted. In the experimental group, 19 cases of 27 implants were selected and the diameter of the healing abutment was changed to 3.8 mm. The conjunctival epithelium was reconstructed, the 3.8 mm prosthesis was repaired, and the porcelain crown with high gold content was repaired. In the control group, 19 cases of 24 implants were treated with healed abutments, and repaired the flush butt connection-type and porcelain-rich porcelain crowns with the same diameter as the implants. X-ray examination was performed on the implant before and after loading in March, June and December respectively to measure and evaluate the bone resorption and soft tissue health of the margins of the implant. Results: The amount of bone resorption at the neck of the platform was significantly lower than that of the implant (P <0.01). There was no significant difference in the soft tissue between the two groups (P> 0.05). CONCLUSIONS: Compared with flush-type docking, the clinical platform conversion design preserves the bone height at the neck of the implant within 1 year of functional loading.
