目的 探讨周期素依赖激酶抑制剂 p2 7蛋白在滋养细胞中的表达及其与滋养细胞疾病发生、发展和预后的关系。方法 采用免疫组织化学 SP(L SAB)法检测 35例滋养细胞中 p2 7蛋白的表达情况。结果 p2 7蛋白在10例正常早孕绒毛和 15例良性葡萄胎中的表达阳性率明显高于 11例侵蚀性葡萄胎和 9例绒毛膜癌 (P<0 .0 5 )。正常早孕绒毛和良性葡萄胎 p2 7蛋白表达阳性率分别为 80 .0 % (8/10 )和 73.3% (11/15 ) ,而 2 0例恶性滋养细胞疾病中 ,仅 4例 p2 7蛋白表达阳性 (2 0 .0 % )。正常早孕绒毛和良性葡萄胎中 p2 7蛋白表达阳性率无显著差异 (P>0 .0 5 )。15例良性葡萄胎中 ,7例子宫大于停经月份者有 3例 p2 7蛋白表达阳性 ,而 8例子宫小于或相当于停经月份者 p2 7蛋白表达均为阳性 ,两者差异有显著性 (P<0 .0 5 )。随访 14例良性葡萄胎患者有 2例恶变 ,且其 p2 7蛋白表达均为阴性。结论 p2 7蛋白表达减少或缺失与滋养细胞疾病的发生、发展和转归有一定相关性 ,检测 p2 7蛋白在滋养细胞中的表达水平有助于良性葡萄胎的预后判断 ,可能用于指导治疗。
Objective To investigate the expression of cyclin-dependent kinase inhibitor p27 in trophoblast cells and its relationship with the occurrence, development and prognosis of trophoblastic diseases. Methods The expression of p27 protein in 35 cases of trophoblastic cells was detected by immunohistochemistry SP (L SAB) method. Results The positive rate of p27 protein in 10 cases of normal early pregnancy villi and 15 cases of benign mole was significantly higher than that of 11 cases of invasive mole and 9 cases of choriocarcinoma (P <0.05). The positive rates of p27 protein expression in normal early pregnancy and benign hydatidiform moles were 80.0% (8/10) and 73.3% (11/15) respectively, while in 20 cases of malignant trophoblastic disease, only 4 cases expressed p27 protein Positive (20.0%). There was no significant difference in the positive rate of p27 protein between normal early pregnancy villi and benign mole (P> 0.05). Among the 15 cases of benign hydatidiform mole, 7 cases expressed more p2 7 protein than 7 cases whose uterus was larger than the menorrhagia month, while 8 cases showed the positive expression of p27 protein (p < <0 .0 5). Two cases of malignant transformation were observed in 14 cases of benign hydatidiform mole, and the expression of p27 protein was negative. Conclusion The decrease or deletion of p27 protein may be related to the occurrence, development and prognosis of trophoblastic diseases. The detection of the expression of p27 in trophoblastic cells is helpful for the prognosis of benign hydatidiform mole and may be used to guide the treatment .
