目的探讨体素内不相干运动(IVIM)理论的多b值磁共振扩散加权成像(DWI)在大鼠放疗后早期肝纤维化(HF)诊断及分期中的价值。方法将30只雌性SD大鼠随机分为3组,对右半肝分别进行1、2、3个月放疗,以制备不同病理分期的放射性大鼠肝纤维化模型。于放疗前、放疗后1、2、3个月分别对SD大鼠进行磁共振T1WI、T2WI、多b值DWI序列扫描,测量放疗前后不同时间点肝脏左右叶ADC、D、D*、f值。在放疗后的每个时间点MRI检查扫描完成后即刻处死大鼠并取病理标本。结果各时间点放疗后与放疗前对比,肝右叶ADC值与D值降低,其差别均有统计学意义(P<0.05);ADC值与D值与HF病理分期呈中度负相关;D*值及f值的差别无统计学意义(P>0.05)。结论放疗后早期HF在常规MRI序列上未见改变,ADC及D值能准确反映早期HF的严重程度,为活体评价早期HF的重要指标。
Objective To investigate the value of Multi-b-value Magnetic Resonance Diffusion Weighted Imaging (DWI) with IVIM in the diagnosis and staging of early liver fibrosis (HF) after radiotherapy in rats. Methods Thirty female Sprague-Dawley rats were randomly divided into three groups. The right half-liver was treated with radiotherapy for 1, 2, and 3 months respectively to prepare radioactive rat hepatic fibrosis model with different pathological stages. Before and after radiotherapy, 1, 2, and 3 months after radiotherapy, MR imaging was performed on T1WI, T2WI and multi-b DWI of SD rats, and the ADC, D, D *, f values at different time points . At each time point after radiotherapy, MRI examination was performed immediately after the completion of the rats were sacrificed and pathological specimens were taken. Results Compared with before radiotherapy, the ADC value and D value of the right lobe of liver decreased at each time point after radiotherapy, and the differences were statistically significant (P <0.05); ADC value and D value were moderately negative correlated with the pathological stage of HF; D There was no significant difference between value and f value (P> 0.05). Conclusion There is no change of routine MRI in the early stage after radiotherapy. The ADC and D values can accurately reflect the severity of early HF, which is an important index for assessing early HF in vivo.
