目的:分析彩色高频多普勒超声检查乳腺癌肿块的特征表现并评价其应用价值。方法:分析经手术后病理证实的65例乳腺癌肿块的彩色多普勒超声声像图特征、彩色血流成像(CDFI)显示的血流分布特点以及频谱多普勒测及血流参数阻力指数(RI)的大小。结果:肿块形态不规则或呈分叶状48例(73.8%);肿块边缘欠清晰或呈蟹足样等征象53例(81.5%);内部回声不均匀56例(86.1%);后方回声衰减32例(49.2%)。另有10例(15.3%)肿块内部伴有沙粒样钙化。彩色多普勒超声CDFI表现:46例(70.7%)肿块血流分布为2~3级,8例(12.3%)血流分布为1级,11例(16.9%)血流分布为0级。PW表现:65例中乳腺癌肿块血流动力学参数RI≥0.7者50例(76.9%)。结论:高频彩色多普勒超声能够准确地反映乳腺癌肿块的特征变现,是鉴别乳腺肿块性质的重要检查方法。
Objective: To analyze the characteristics of breast cancer mass by color Doppler ultrasonography and to evaluate its application value. Methods: The color Doppler ultrasonographic features of 65 cases of breast cancer confirmed by pathology were analyzed. The distribution of blood flow showed by color flow imaging (CDFI) and the index of resistance of flow parameters (RI) size. Results: There were 48 cases (73.8%) with irregular or lobulated lumps, 53 cases (81.5%) with unclear margin or crab-like signs, 56 cases (86.1%) with internal echo unevenness, 32 cases (49.2%). Another 10 cases (15.3%) mass with calcified calcification inside. Color Doppler ultrasound CDFI performance: 46 cases (70.7%) mass flow distribution of 2 to 3, 8 cases (12.3%) blood flow distribution of grade 1, 11 cases (16.9%) blood flow distribution of 0. PW performance: 65 cases of breast tumor hemodynamic parameters RI ≥ 0.7 in 50 cases (76.9%). Conclusion: High-frequency color Doppler ultrasound can accurately reflect the characteristics of breast cancer lumps, is an important check to identify the nature of breast lumps.
