目的:探讨磁共振成像(MRI)动态增强联合扩散加权成像(DWI)在子宫内膜癌诊断中的应用价值。方法:回顾性分析2019年2月至2020年2月于濮阳市油田总医院就诊的82例高度疑似子宫内膜癌患者的临床资料,全部患者均接受MRI动态增强、DWI成像及手术病理学检查,并记录其诊断结果。以病理结果为金标准,分析两种成像单独及联合诊断与金标准的一致性,并分析其诊断效能。结果:82例高度疑似子宫内膜癌患者中,手术病理结果显示,58例确诊为子宫内膜癌,检出率为70.73%;MRI动态增强、DWI成像单独及联合诊断子宫内膜癌与金标准的一致性n Kappa值为分别为0.574、0.417、0.814,相较于MRI动态增强、DWI成像单独诊断,联合诊断与金标准的一致性更好。联合诊断的准确度(91.46%)、敏感度(98.28%)、阴性预测值(95.00%)均高于单纯DWI成像(74.39%、77.59%、55.17%)和MRI动态增强成像(81.71%、84.48%、66.67%),差异有统计学意义(n P<0.05)。n 结论:MRI动态增强联合DWI成像用于子宫内膜癌诊断的准确度、灵敏度更高,且与病理结果的一致性更好,应用价值极高。“,”Objective:To investigate the value of dynamic contrast-enhanced magnetic resonance imaging (MRI) combined with diffusion weighted imaging (DWI) in the diagnosis of endometrial carcinoma.Methods:The clinical data of 82 patients with highly suspected endometrial carcinoma who were treated in Puyang Oilfield General Hospital from February 2019 to February 2020 were retrospectively analyzed. All patients underwent dynamic contrast-enhanced MRI, DWI, and pathological examination. And the diagnosis results were recorded. With surgical pathological results as the gold standard, the consistency and diagnostic efficiency of dynamic contrast-enhanced MRI, DWI, and combination of them were analyzed.Results:Among 82 patients with highly suspected endometrial carcinoma, 58 cases were diagnosed with endometrial carcinoma by pathological examination, and the detection rate was 70.73%. The consistency n Kappa value of dynamic contrast-enhanced MRI, DWI, and combination of them with the gold standard, in the diagnosis of endometrial carcinoma, was 0.574, 0.417 and 0.814, respectively. Compared with dynamic contrast-enhanced MRI, and DWI, the combination of them has better consistency with the gold standard. The accuracy (91.46%), sensitivity (98.28%) and negative predictive value (95.00%) of combined diagnosis were higher than those of DWI alone (74.39%, 77.59%, 55.17%), and dynamic contrast-enhanced MRI (81.71%, 84.48%, 66.67%), n P<0.05.n Conclusions:Dynamic contrast-enhanced MRI combined with DWI has a higher accuracy and positive predictive value in the diagnosis of endometrial carcinoma. It has better consistency with pathological results, and has great application value.
