目的:观察多西他赛与环磷酰胺、表阿霉素、氟脲嘧啶(CEF方案)新辅助化疗方案治疗局部晚期乳腺癌的临床疗效和不良反应。方法:经空芯针穿刺确诊局部晚期乳腺癌患者17例,随机分为多西他赛组(7例),CEF组(10例),分别给予多西他赛方案,环磷酰胺、表阿霉素、氟脲嘧啶方案化疗3个周期,化疗结束2周后行乳腺癌改良根治术。比较2组疗效和不良反应,治疗前、后TNM分期变化,及不同激素受体患者的疗效。结果:2组临床疗效比较多西他赛组优于CEF组,但差异无统计学意义(P>0.05);2组TNM分期均较新辅助化疗前降低(P<0.05),不良反应比较差异无统计学意义(P>0.05);雌激素受体阴性者疗效优于雌激素受体阳性者(P<0.05),孕激素受体阴性者疗效优于孕激素受体阳性者(P<0.05)。结论:多西他赛与环磷酰胺、表阿霉素、氟脲嘧啶新辅助化疗方案治疗局部晚期乳腺癌安全有效,激素受体阴性者疗效优于阳性者。 Objective: To observe the clinical efficacy and adverse reactions of docetaxel and cyclophosphamide, epirubicin and fluorouracil (CEF regimen) neoadjuvant chemotherapy in the treatment of locally advanced breast cancer. Methods: Seventeen patients with locally advanced breast cancer were diagnosed by air-core needle aspiration. They were randomly divided into docetaxel group (n = 7) and CEF group (n = 10), and were given docetaxel, cyclophosphamide, 3 cycles of chemotherapy with neomycin and 5-fluorouracil, and modified radical mastectomy of breast cancer 2 weeks after the end of chemotherapy. The efficacy and adverse reactions of two groups were compared, TNM staging changes before and after treatment, and the efficacy of different hormone receptor patients. Results: The clinical efficacy of the two groups was better than that of the CEF group, but the difference was not statistically significant (P> 0.05). The TNM staging of the two groups was lower than that before neoadjuvant chemotherapy (P <0.05), and the differences were statistically significant (P <0.05). The effect of estrogen receptor-negative patients was better than that of estrogen receptor-positive patients (P <0.05), but the effect of progesterone receptor negative patients was better than that of progesterone receptor-positive patients (P <0.05) ). Conclusion: Docetaxel is safe and effective with cyclophosphamide, epirubicin and fluorouracil neoadjuvant chemotherapy in the treatment of locally advanced breast cancer. Hormone receptor negative patients are superior to those with positive results.
