目的探讨胃十二指肠溃疡急性穿孔的诊断和治疗方法。方法对2004年1月至2008年5月收治的26例胃十二指肠溃疡急性穿孔患者的临床资料进行回顾性分析。结果2例保守治疗,24例行手术治疗。其中14例行单纯穿孔修补术,12例行胃大部切除术。死亡1例,治愈率96.2%。结论胃十二指肠溃疡穿孔修补术和胃大部切除术是治疗胃十二指肠溃疡穿孔的常用术式。溃疡穿孔修补+扩大壁细胞的迷走神经切除术是较好的手术方式,值得临床推广应用。 Objective To investigate the diagnosis and treatment of acute perforation of gastroduodenal ulcer. Methods The clinical data of 26 patients with acute perforation of gastroduodenal ulcer admitted from January 2004 to May 2008 were analyzed retrospectively. Results of 2 cases of conservative treatment, 24 cases were treated surgically. Among them, 14 cases underwent simple perforation repair and 12 cases underwent subtotal gastrectomy. 1 case died, the cure rate was 96.2%. Conclusion Gastro-duodenal ulcer perforation repair and subtotal gastrectomy are common procedures for the treatment of perforation of gastroduodenal ulcer. Ulcer perforation repair + expansion of parietal cells of the vagotomy is a better surgical approach, it is worth promoting the clinical application.
