目的利用无创心功能监测仪研究呼气末正压(PEEP)对血流动力学影响。方法机械通气后监测不同PEEP水平心功能指标,包括中心静脉压(CVP)、心率(HR)、平均动脉压(MAP)、心脏排血指数(CI)、胸液含量(TFC)。结果随着PEEP水平增加,CVP增加,CI降低,MAP下降,TFC减少,HR无变化。结论机械通气应用PEEP会影响血流动力学稳定,应监测相关血流动力学指标,权衡利弊。
Objective To study the hemodynamic effects of positive end expiratory pressure (PEEP) using a non-invasive cardiac function monitor. Methods Cardiac function indexes of different PEEP levels were monitored after mechanical ventilation, including CVP, HR, MAP, cardiac perfusion index (CI) and pleural fluid content (TFC). Results With the increase of PEEP level, CVP increased, CI decreased, MAP decreased, TFC decreased, HR did not change. Conclusions The application of PEEP in mechanical ventilation may affect the hemodynamic stability. We should monitor the relevant hemodynamic parameters and weigh the pros and cons.
