目的观察优质护理服务预防人工关节置换术后感染的效果。方法选择2014年2月—2016年5月收治的关节置换术患者650例,将患者随机分为对照组与干预组,每组各325例。对照组给予常规护理,干预组给予优质护理服务。对比观察两组切口愈合情况;术后3个月,记录两组假体周围感染情况,检测两组白细胞计数、血沉及CRP;出院时统计两组护理满意度。计量资料比较采用t检验,计数资料比较采用χ~2检验,P<0.05为差异有统计学意义。结果干预组甲级愈合率96.31%高于对照组(85.85%),两组比较差异有统计学意义(P<0.05)。术后3个月,观察组发生亚急性假体周围感染8例(2.46%),干预组发生亚急性假体周围感染1例(0.31%),两组比较差异有统计学意义(P<0.05)。两组白细胞计数、血沉比较差异无统计学意义(P>0.05);干预组CRP[(5.03±2.34)mg/L]低于对照组[(9.06±2.17)mg/L],比较差异有统计学意义(P<0.05)。对照组满意度为84.92%,干预组满意度为97.23%,两组比较差异有统计学意义(P<0.05)。结论优质护理服务可预防人工关节置换术后感染,提高患者满意度。
Objective To observe the effect of quality nursing service to prevent infection after artificial joint replacement. Methods A total of 650 patients with arthroplasty who were treated from February 2014 to May 2016 were randomly divided into control group and intervention group, with 325 patients in each group. The control group was given routine nursing, and the intervention group was given quality nursing service. The wound healing was compared between the two groups. After 3 months, the infection around the prosthesis was recorded. The white blood cell count, erythrocyte sedimentation rate and CRP were measured in both groups. At discharge, the nursing satisfaction was calculated. Measurement data were compared using t test, count data were compared using χ ~ 2 test, P <0.05 for the difference was statistically significant. Results The grade A healing rate in the intervention group was 96.31% higher than that in the control group (85.85%). There was significant difference between the two groups (P <0.05). Three months after operation, 8 cases (2.46%) had peripheral subacute prosthesis infection in observation group and 1 case (0.31%) had subacute prosthesis infection in intervention group, the difference was statistically significant (P <0.05 ). There was no significant difference in erythrocyte count and erythrocyte sedimentation rate between the two groups (P> 0.05). CRP (5.03 ± 2.34) mg / L in the intervention group was lower than that in the control group (9.06 ± 2.17 mg / L) Significance (P <0.05). The satisfaction of the control group was 84.92%, and the satisfaction rate of the intervention group was 97.23%. There was significant difference between the two groups (P <0.05). Conclusion High quality nursing services can prevent infection after artificial joint replacement and improve patient satisfaction.
