本实验根据门脉高压巨脾病理分级标准,挑选纤维化巨脾Ⅰ~Ⅲ级各一例。于H-500电镜下观察其超微结构改变,主要观察胶原纤维的增生分布特点,脾脏窦壁上内皮、基膜和网状细胞的改变,具有吞噬功能的各种细胞吞噬现象的改变和某些亚细胞结构的改变。发现:Ⅰ级巨脾以淤血为主,纤维性增生较轻,但仍有反映脾脏良好吞噬滤过功能的组织形态:Ⅱ级巨脾纤维化较为明显,胶原向脾索窦壁延伸,致窦壁三层结构显著改变,吞噬细胞的某些亚细胞结构异常:Ⅲ级巨脾显示严重的纤维化,胶原广泛分布,某些正常的脾脏结构消失,视野内难以发现巨噬细胞和浆细胞。
According to the grading criteria of portal hypertension splenomegaly, we selected one case of grade Ⅰ ~ Ⅲ fibrosis. Observed by H-500 electron microscopy ultrastructural changes, the main observation of the proliferation of collagen fibers distribution, changes in the endothelium, basement membrane and reticular cells in the splenic sinus wall, phagocytosis with a variety of phagocytosis and a Some subcellular structure changes. The results showed that the splenomegaly of grade Ⅰ splenomegaly was mainly fibrosis, fibrogenesis was mild, but there was still some morphological changes reflecting good phagocytosis of spleen: the splenic fibrosis of grade Ⅱ spleen was more obvious, Wall three-tier structure changed significantly, some of the subcellular structure of phagocytes abnormalities: Ⅲ splenomegaly showed severe fibrosis, collagen is widely distributed, some of the normal spleen structure disappears, difficult to find macrophages and plasma cells in the field of vision.
