目的探讨术前超声对甲状腺癌颈部淋巴结手术方式选择的临床价值。方法回顾性分析267例分化型甲状腺癌患者的临床资料,将其颈部淋巴结术前超声所见与手术方式及术后病理进行对照研究。结果术前超声诊断颈部淋巴结转移诊断的准确度88.0%、灵敏度85.4%、特异度91.4%。甲状腺癌颈部淋巴结转移区域多见于颈旁区(Ⅱ~Ⅳ区,43.1%)及中央区(Ⅵ区,37.5%),显著高于Ⅰ区(1.1%)及V区(5.2%),其差异存在显著的统计学意义(P<0.01)。且术前超声在颈部不同分区或不同手术清扫方式中对甲状腺癌颈部淋巴结转移的诊断与病理均有较高的符合率。结论术前超声检查可对甲状腺癌颈部淋巴结转移做出比较客观的准确诊断,对手术方案的选择具有一定的指导意义。
