目的探讨联合超声下未成熟卵泡抽吸术(IMFA)与二甲双胍对氯米酚抵抗的多囊卵巢综合征(PCOS)患者行促排卵前预处理对其卵巢反应性及治疗结局改善的有效性。方法 92例氯米酚抵抗的PCOS患者随机分成对照组和IMFA组,每组46例。对照组服用二甲双胍及口服避孕药预处理,IMFA组在口服药物的基础上进行未成熟卵泡抽吸术,比较两组促排卵治疗结局。结果两组预处理后IMFA组促性腺激素(Gn)量低于对照组,单卵泡发生率较对照组高(P<0.05)。两组妊娠情况差异无统计学意义(P>0.05),但IMFA组妊娠率升高,流产率降低。结论 IMFA可以有效改善氯米芬抵抗的PCOS患者的卵巢反应性,IMFA联合二甲双胍和口服避孕药在氯米芬抵抗的PCOS的治疗中提供了有效的新途径。
Objective To investigate the effectiveness of pre-ovulation pretreatment on the ovarian response and therapeutic outcome in patients with polycystic ovary syndrome (PCOS) who underwent immature follicle aspiration (IMFA) and metformin-resistant clomiphene resistance under combined ultrasound. Methods A total of 92 PCOS patients resistant to chlorphenamine were randomly divided into control group and IMFA group, 46 cases in each group. Control group was treated with metformin and oral contraceptive pretreatment, IMFA group on the basis of oral medication for immature follicle aspiration, the treatment of ovulation in both groups was compared. Results The amount of gonadotropin (Gn) in IMFA group was lower than that in control group after pretreatment. The incidence of single follicle in control group was higher than that in control group (P <0.05). There was no significant difference in pregnancy between the two groups (P> 0.05), but the pregnancy rate in IMFA group was increased and the abortion rate was decreased. Conclusion IMFA can effectively improve the ovarian response in clomiphene-resistant PCOS patients. IMFA combined with metformin and oral contraceptives provide an effective new approach in the treatment of clomiphene-resistant PCOS.
