长期应用脂乳剂于全静脉营养(TPN)可损害肝内胆红素转移机理,导致进行性胆汁郁积。如不及时纠正,所造成的损害常是进行性的、不可逆的肝细胞坏死和门脉周周纤维化。本文报告35例病人接受TPN3周后肝功能的变化。病人共分5组:组1、3、5为高脂摄入(每天按体重给脂乳剂3g/kg),共18名,平均39天。组2、4为中量脂摄入(每天给1g/kg),共17名,平均37.1天。各组病人的TPN包含的其它营养成份为氨基酸,组1、22g/kg,组3、4减为1g/kg,组5则减少葡萄糖供热卡的份额。使各组液体摄入量均为3,000
Long-term application of lipid emulsion in total parenteral nutrition (TPN) can damage the intrahepatic bilirubin transfer mechanism, leading to progressive cholestasis. If not corrected in time, the damage caused is often progressive, irreversible liver cell necrosis and peripheral fibrosis. This article reports 35 patients after receiving TPN 3 weeks change in liver function.
