目的分析血栓性血小板减少性紫癜(TTP)的误诊原因及治疗方法。方法通过报告8例TTP误诊患者的临床诊治过程,结合文献对TTP的发病机制、临床表现、诊断治疗等进行探讨,研究其误诊原因。结果 8例患者初诊均误诊,死亡4例,存活3例,自动出院1例。确诊手段均为:外周血涂片检查,其中6例并行骨髓涂片检查。结论 TTP的临床表现呈多样化,极易被误诊,血浆置换治疗应被早期应用。
Objective To analyze the causes of misdiagnosis and treatment of thrombotic thrombocytopenic purpura (TTP). Methods Eight cases of TTP misdiagnosed patients were reviewed. The pathogenesis, clinical manifestations, diagnosis and treatment of TTP were discussed in combination with the literature, and the causes of misdiagnosis were studied. Results All 8 patients were misdiagnosed as initial diagnosis, 4 died, 3 survived and 1 patient was discharged automatically. Diagnostic methods are: peripheral blood smears, of which 6 cases of parallel bone marrow smears. Conclusion The clinical manifestation of TTP is diversified and easily misdiagnosed. Plasma exchange should be used early.
