目的分析原发性高血压(EH)患者血清炎性因子脂蛋白相关磷脂酶A2(LP-PLA2)与缺血性卒中的关系,并探讨其临床意义。方法回顾性连续纳入符合纳入标准的EH患者167例,按照靶器官损伤(TOD)诊断标准分为单纯EH组79例,EH合并卒中组88例。测定每组入选对象的LP-PLA_2水平,比较各组间临床相关资料和LP-PLA_2水平的差异。采用多因素Logistic回归分析探讨LP-PLA_2水平对EH患者发生缺血性卒中的临床意义。结果 Logistic回归分析结果显示,LP-PLA_2是EH患者发生缺血性卒中的独立危险因素(OR=13.699,95%CI:5.236~35.837,P<0.01);在入选的危险因素中,LP-PLA_2的标化偏回归系数b’绝对值最大(b’=0.708)。结论 LP-PLA_2作为新型的炎性标记物,是缺血性卒中重要的独立危险因素,对EH患者发生缺血性卒中的影响最大,可能为脑血管事件的预防和治疗提供新方向。
Objective To analyze the relationship between serum inflammatory factor lipoprotein-associated phospholipase A2 (LP-PLA2) and ischemic stroke in patients with essential hypertension (EH) and to explore its clinical significance. Methods A total of 167 consecutive EH patients were retrospectively included. According to the target organ damage (TOD) criteria, 79 patients were divided into EH group and 88 patients with EH combined with stroke group. The level of LP-PLA 2 in each group was measured, and the differences of the clinical data and LP-PLA 2 levels between the groups were compared. Multivariate Logistic regression analysis was used to investigate the clinical significance of LP-PLA 2 level in ischemic stroke patients with EH. Results Logistic regression analysis showed that LP-PLA_2 was an independent risk factor for ischemic stroke in patients with EH (OR = 13.699, 95% CI: 5.236-35.837, P <0.01). Among the risk factors for inclusion, LP- The normalized partial regression coefficient b ’has the largest absolute value (b’ = 0.708). Conclusions LP-PLA2, as a novel inflammatory marker, is an important independent risk factor for ischemic stroke and has the greatest impact on ischemic stroke in patients with EH. It may provide a new direction for the prevention and treatment of cerebrovascular events.
