乌克兰的历史决定了东西部地区具有不同的特点,乌克兰东部地区受前苏联法律的深刻影响,而西部地区更具有欧洲的特点。该研究通过大范围的调查,比较乌克兰在医院效益方面的近期发展趋势,分析这种不同的社会、文化特点是否会对医院的效益产生明显影响。在前苏联模式计划经济刚性的前提下,西部地区由于具有较高的管理和医疗技术水平,从而在经济效益方面体现出较快的发展趋势。该研究的数据来自三个州,其中一个属于西部地区,两个属于东部地区,调查跨度从1997年到2001年。应用数据包络分析方法来估计医院的技术效率。校正偏倚后,进行两阶段的Tobit回归分析。结果显示西部地区医院的效益提高,而东部地区的医院却保持在一个比较恒定的水平。西部地区能够较好地适应管理和医疗商业实践活动,较快地吸收一些新的技术去提高卫生服务效率,而在东部地区个人决策和企业由于国家推崇中央决策而受到抑制。
The history of Ukraine determines the different characteristics of the eastern and western regions. The eastern part of Ukraine is deeply influenced by the laws of the former Soviet Union, while the western region is more European. Through a large-scale survey, the study compares Ukraine’s recent trends in hospital effectiveness and analyzes whether such different social and cultural characteristics have a significant impact on the hospital’s effectiveness. Under the premise of planned economic rigidity of the former Soviet Union model, the western region has a relatively fast development trend in terms of economic benefits due to its high level of management and medical technology. The data for this study come from three states, one of which belongs to the western region and two belong to the eastern region, with a span of investigation from 1997 to 2001. Apply data envelopment analysis to estimate the technical efficiency of hospitals. After correcting for bias, a two-stage Tobit regression analysis was performed. The results showed that the efficiency of hospitals in the western region increased, while the hospitals in the eastern region maintained a relatively constant level. The western region can better adapt to management and medical business practices, quickly absorb new technologies to improve the efficiency of health services, while individual decisions and enterprises in the eastern region are suppressed due to the country’s esteem of the central government’s decision-making.
