目的探讨正电子发射体层摄影/计算机体层摄影(PET/CT)18氟脱氧葡萄糖(18F-FDG)显像、磁共振显像(MRI)及两种方法联合应用对胰腺良恶性病变的诊断价值。方法回顾性分析在我院先后行18F-FDG PET/CT和MRI检查的胰腺病变(时间间隔≤2周),并以病理及临床影像随访(随访时间≥6个月)结果作为金标准,比较18FFDG PET/CT、MRI及两者联合应用对胰腺良恶性病变诊断的灵敏性、特异性、准确性、阳性预测值及阴性预测值。结果共86个病灶,其中58个恶性,28个良性,18F-FDG PET/CT的诊断灵敏性、特异性、准确性、阳性预测值及阴性预测值分别为86.21%,82.14%,84.88%,90.91%及74.19%,MRI分别为67.24%,64.29%,66.28%,79.59%及48.65%,PET/CT+MRI分别为93.10%,89.29%,91.86%,94.74%及86.21%,3种方法的灵敏性、准确性、阳性预测值及阴性预测值比较差异有统计学意义(P均<0.05),特异性比较差异无统计学意义(P>0.05)。结论18F-FDG PET/CT显像对胰腺良恶性病变的诊断价值较MRI高,必要时两者联合应用可以提高对胰腺病变的诊断价值。
Objective To investigate the diagnostic value of positron emission tomography / computed tomography (18F-FDG) 18 fluorodeoxyglucose (18F-FDG) imaging and magnetic resonance imaging (MRI) in the diagnosis of benign and malignant pancreatic lesions value. Methods We retrospectively analyzed the 18F-FDG PET / CT and MRI examinations of pancreatic lesions (interval ≤ 2 weeks) in our hospital. The results of pathology and clinical imaging follow-up (≥6 months) were used as gold standard. Sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 18FFDG PET / CT, MRI and the combination of the two in the diagnosis of benign and malignant pancreatic lesions. Results A total of 86 lesions were detected. The diagnostic sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 86 malignant and 28 benign, 18F-FDG PET / CT were 86.21%, 82.14% and 84.88% 90.91% and 74.19% respectively, and MRI was 67.24%, 64.29%, 66.28%, 79.59% and 48.65% respectively, and PET / CT + MRI were 93.10%, 89.29%, 91.86%, 94.74% and 86.21% Sensitivity, accuracy, positive predictive value and negative predictive value were significantly different (P <0.05). There was no significant difference in specificity (P> 0.05). Conclusion The diagnostic value of 18F-FDG PET / CT imaging in benign and malignant pancreatic lesions is higher than that of MRI. If necessary, the combination of both can improve the diagnostic value of pancreatic lesions.
