目的探讨早期高压氧治疗对重型颅脑损伤患者的临床疗效。方法选取2005-01-2010-01 104例在我院神经外科住院的重型颅脑损伤的患者,根据患者病情给予手术、脱水、止血、抗生素、脑细胞营养药、适量皮质激素、促醒药物及头部降温治疗,伴有精神症状者给予适量镇静药或冬眠疗法,病情平稳后1~2周内开始行高压氧治疗的患者57例(观察组),病情平稳后3~4周开始行高压氧治疗的47例(对照组)。结果观察组的患者生存率和愈后功能恢复率均高于对照组(P<0.01),差异有统计学意义。结论重型颅脑损伤应用高压氧治疗越早越好,可提高脑组织的有氧代谢,降低颅内压,认真做好治疗期的护理,严防并发症的发生,从而使重型颅脑损伤患者的生存率和功能恢复率均得到提高。
Objective To investigate the clinical effects of early hyperbaric oxygen therapy on patients with severe craniocerebral injury. Methods A total of 104 patients with severe craniocerebral injury who were hospitalized in Department of Neurosurgery from January 2005 to October 2010 were enrolled. According to the patients’ condition, they were given surgery, dehydration, hemostasis, antibiotics, brain cell nutrition drugs, proper amount of corticosteroids, Head cooling treatment, accompanied by psychiatric symptoms were given sedatives or hibernation therapy, stable condition within 1 to 2 weeks after the beginning of hyperbaric oxygen therapy in 57 patients (observation group), 3 to 4 weeks after the onset of stable high pressure Oxygen therapy in 47 cases (control group). Results The survival rate of the patients in the observation group and the recovery rate of the function after healing were higher than those in the control group (P <0.01), the difference was statistically significant. Conclusion The treatment of severe craniocerebral injury with hyperbaric oxygen is as soon as possible, which can improve the aerobic metabolism of brain tissue, reduce intracranial pressure, conscientiously do the treatment of the treatment period and prevent the occurrence of complications, so that the patients with severe craniocerebral injury Survival and functional recovery rates have been improved.
