目的:探讨前列腺等离子电切术灌注液高度对患者血流动力学、出血量和手术时间的影响。方法:采用临床随机对照试验法,将180例入选前列腺增生电切患者用随机数字表发分为三组(每组60例),观察在不同灌注高度(40 cm、60 cm和80 cm)下三组患者术中不同血流动力学、出血量和手术时间的变化。结果:采用80 cm灌注高度时术中平均动脉压(MAP)及心率(HR)较术前明显提高,血氧饱和度(SPO2)和术前相比明显降低(P<0.05)。采用40 cm和60 cm灌注高度时术中MAP、HR及SPO2与术前相比无明显变化(P>0.05)。40 cm灌注高度患者的手术时间和平均出血量明显高于其他两组(P<0.05);60 cm和80 cm灌注高度两组之间无明显差异。结论:60 cm灌注高度时既可以保证视野清晰,缩短手术时间,减少出血量,同时对患者血流动力学无明显改变,是前列腺等离子电切时最佳的灌洗高度。
Objective: To investigate the effects of prostatic plasma tamponade perfusion height on hemodynamics, blood loss and operative time. Methods: A randomized controlled clinical trial was conducted in 180 patients with prostatic hyperplasia who were divided into three groups (60 in each group) by random number table. The effects of different perfusion levels (40 cm, 60 cm and 80 cm) were observed Three groups of patients during the operation of different hemodynamics, blood loss and operation time changes. Results: Mean arterial pressure (MAP) and heart rate (HR) at 80 cm perfusion height were significantly higher than those before operation, while oxygen saturation (SPO2) was significantly lower than that before operation (P <0.05). The MAP, HR and SPO2 in operation at 40 cm and 60 cm did not change significantly compared with that before operation (P> 0.05). The operation time and average blood loss of patients with 40 cm perfusion height were significantly higher than those of the other two groups (P <0.05). There was no significant difference between the two groups at 60 cm and 80 cm perfusion height. Conclusion: 60 cm perfusion height can not only ensure a clear field of vision, shorten the operation time and reduce the amount of bleeding, while no significant changes in the patient hemodynamics, is the best lavage height prostatectomy.
