检测糖尿病患者血浆血管内皮生长因子 (vascularendothelial growth factor,VEGF)的含量并探讨它与糖化血红蛋白 (glycosylated hemoglobinc,GHb Alc)、微量白蛋白尿等的关系及在糖尿病视网膜病变 (diabetic retinopathy,DR)发生发展中的作用。方法 :收集 74例 型糖尿病患者和 1 8例正常人的血浆标本 ,检测血中 VEGF、GHb Alc、FBS和尿中微量白蛋白 (microalbumin,ALB)、肌酐 (Cr)尿微量白蛋白指数(urinary microalbumin excretion index,ALB/ Cr,or A/ C)等。结果 :1 .VEGF、ALB、A/ C在对照组、无视网膜病变 (non- diabetic retinopathy,NDR)组、背景型 DR(background- DR,BDR)组中逐渐升高 ,BDR组的 VEGF比对照组显著增高 ,增生型 DR(proliferative- DR,PDR)组的 ALB、A/ C比 NDR组显著增高 ;2 .正常人群检测不到 ALB(<5mg/ ml) ,糖尿病患者各组中能不同程度的检出 ALB,甚至超出仪器能测出的最高值 (>30 0 mg/ ml) ,各组间的检出率有显著差异 ;3.VEGF与 GHb Alc、ALB及 A/ C成正相关 ,与年龄、病程、FBS、Cr等均无相关性。结论 :VEGF在糖尿病微血管病变的发生发展中起着重要作用。
