目的:观察急性胰腺炎(AP)患者Bregs细胞表达的变化,并探讨了其相关的临床意义。方法:收集2014-11-2016-07我院诊治的AP患者78例,另选取我院体检中心健康体检者40例对照。流式细胞技术检测Bregs细胞比例变化,Elisa法检测IL-10表达。结果:AP患者Bregs细胞比例为(13.74±2.47)%,显著低于对照组的(21.16±4.35)%(P<0.01),AP患者IL-10表达为(16.62±3.54)ng/ml,显著低于对照组的(27.48±5.62)ng/ml(P<0.01)。患者入院后第1、5、7天急性重症胰腺炎(SAP)患者Bregs细胞和IL-10表达均显著低于急性轻型胰腺炎(MAP)患者(P<0.05或P<0.01),MAP患者Bregs细胞和IL-10表达入院后第1天到第3和第7天逐渐升高(P<0.05),而SAP患者Bregs细胞和IL-10表达入院后第1天到第3天和第7天逐渐减低(P<0.05)。Pearson线性相关分析显示AP患者Bregs细胞与APACHE评分呈显著负相关(r=-0.412,P<0.05),IL-10表达也与APACHE评分呈显著负相关(r=-0.424,P<0.05)。结论:AP患者Bregs细胞和IL-10低表达,Bregs细胞和IL-10检测在预测AP严重程度和病情发展方面具有一定的临床意义。
OBJECTIVE: To observe the changes of Bregs in patients with acute pancreatitis (AP) and to explore its clinical significance. Methods: A total of 78 patients with AP who were diagnosed and treated in our hospital from 2014-11-2016-07 were enrolled. Another 40 healthy people were selected as physical examination center in our hospital. Flow cytometry was used to detect the percentage of Bregs cells. Elisa method was used to detect the expression of IL-10. Results: The percentage of Bregs in AP patients was (13.74 ± 2.47)%, significantly lower than that in control group (21.16 ± 4.35)% (P <0.01). The expression of IL-10 in AP patients was (16.62 ± 3.54) ng / Lower than the control group (27.48 ± 5.62) ng / ml (P <0.01). The expression of Bregs and IL-10 in patients with acute severe pancreatitis (SAP) on the 1st, 5th, 7th day after admission were significantly lower than those in patients with acute mild pancreatitis (MAP) (P <0.05 or P <0.01) The expression of IL-10 and the expression of IL-10 gradually rose from the first day to the third day and the seventh day after admission (P <0.05), whereas the expression of Bregs cells and IL-10 in SAP patients were increased from day 1 to day 3 and day 7 after admission Gradually decreased (P <0.05). Pearson linear correlation analysis showed that there was a significant negative correlation between Bregs cells and APACHE score in AP patients (r = -0.412, P <0.05), and negative correlation between IL-10 expression and APACHE score (r = -0.424, P <0.05). Conclusion: The expression of Bregs and IL-10 in AP patients is low, and the detection of Bregs cells and IL-10 has certain clinical significance in predicting the severity of AP and the progression of the disease.
