目的探讨动态增强MRI在乳腺良恶性病变鉴别与诊断中的应用价值。方法选取2013年1月至2014年6月间武汉科技大学孝感市中心医院收治的乳腺病变患者的临床影像学资料80例,以手术病理诊断为金标准,采用专用软件标记信号强度时间曲线,观察病灶形态及边缘情况,统计边缘毛刺、光滑情况及早期增强率。结果经手术病理诊断,良性病变35例,恶性病变45例。其中,良性病变者毛刺状及边缘不规则者少于恶性病变者;恶性病变者A型明显多于良性病变人群,良性病变者MRI增强率以<60%为主,恶性病变者以≥60%为主,两者比较,差异有统计学意义(P<0.05)。结论动态增强MRI有利于鉴别乳腺良恶性病变,同时信号强度时间曲线和早期增强率可辅助临床疾病诊断。
Objective To investigate the value of dynamic contrast-enhanced MRI in the differential diagnosis of benign and malignant breast lesions. Methods Eighty patients with breast lesions who were treated in Xiaogan Central Hospital of Wuhan University of Science and Technology from January 2013 to June 2014 were selected. The pathological diagnosis was taken as the gold standard. The signal intensity time curve was marked by special software and observed Morphology and margins of the lesion, statistical edge burr, smooth condition and early enhancement rate. Results The pathological diagnosis of benign lesions in 35 cases, 45 cases of malignant lesions. Among them, patients with benign lesions had less burr and irregular edges than those with malignant lesions. Type A in malignant lesions was significantly more than those with benign lesions. The rate of MRI enhancement in benign lesions was less than 60%, and those with malignant lesions were ≥60% Mainly between the two, the difference was statistically significant (P <0.05). Conclusions Dynamic contrast-enhanced MRI is helpful to differentiate benign and malignant breast lesions. At the same time, the signal intensity time curve and the early enhancement rate can be used to diagnose the clinical disease.
