目的探讨下咽鳞状细胞癌(hypopharyngeal squamous cell carcinoma,HSCC)中ΔNp63和上皮间质转化(epithelial-mesenchymal transition,EMT)相关分子标志物表达的相关性及临床意义。方法收集第二军医大学长海医院耳鼻咽喉头颈外科2013年1月至2016年11月收治的98例HSCC患者的肿瘤组织及癌旁正常组织标本。采用免疫组织化学法检测肿瘤组织、癌旁正常组织以及肿瘤不同区域中ΔNp63、E-钙黏蛋白(E-cadherin)、波形蛋白(vimentin)的表达,采用Spearman等级相关检验分析各蛋白表达强度的相关性,采用秩和检验比较各蛋白表达强度与患者临床病理特征的关系。结果在HSCC组织中ΔNp63、vimentin的表达强度高于癌旁正常上皮组织(P<0.01),而E-cadherin的表达强度低于癌旁正常上皮组织(P<0.01)。与肿瘤中央区相比,肿瘤边缘区可见ΔNp63、E-cadherin的阳性表达强度下降,vimentin的阳性表达强度上升(P均<0.05)。在HSCC组织中,ΔNp63与E-cadherin的表达强度呈正相关(rs=0.409,P<0.01),与vimentin的表达强度呈负相关(rs=-0.333,P<0.05)。ΔNp63的表达强度与肿瘤T分期、有无淋巴结转移、病理分化程度及临床分期等临床病理特征均无关。E-cadherin和vimentin的表达强度与病理分化程度及有无淋巴结转移有关(P<0.05)。结论ΔNp63的异常表达与HSCC的发生、发展有关,其对EMT可能具有抑制作用。
Objective To investigate the correlation and clinical significance of the expression of ΔNp63 and epithelial-mesenchymal transition (EMT) related molecular markers in hypopharyngeal squamous cell carcinoma (HSCC). Methods 98 cases of HSCC from January 2013 to November 2016 were collected from Department of Otorhinolaryngology Head and Neck Surgery, Changhai Hospital, Second Military Medical University. Immunohistochemistry was used to detect the expression of ΔNp63, E-cadherin and vimentin in tumor tissues, adjacent normal tissues and different tumor areas. Spearman rank correlation test was used to analyze the expression of each protein Correlation, using rank sum test to compare the expression of each protein and clinicopathological features. Results The expression of ΔNp63 and vimentin in HSCC tissues was higher than that in adjacent normal epithelium tissues (P <0.01), while the expression of E-cadherin was lower than that in adjacent normal epithelium tissues (P <0.01). Compared with the central tumor, ΔNp63 was found in the marginal zone of tumor, the positive expression of E-cadherin was decreased and the positive expression of vimentin was increased (all P <0.05). In HSCC, the expression of ΔNp63 was positively correlated with the expression of E-cadherin (rs = 0.409, P <0.01), and negatively correlated with the expression of vimentin (rs = -0.333, P <0.05). The expression intensity of ΔNp63 was not associated with the clinical stage of tumor T stage, lymph node metastasis, pathological differentiation and clinical stage. The expression of E-cadherin and vimentin correlated with the degree of pathological differentiation and lymph node metastasis (P <0.05). Conclusion The abnormal expression of ΔNp63 is related to the occurrence and development of HSCC and may have an inhibitory effect on EMT.
