目的 系统评价孕早期维生素D缺乏与早产结局的相关性.方法 计算机检索PubMed、Embase、Cochrane图书馆、Web of Science、Ebsco、中国生物医学文献数据库、中国知网、万方数据库,搜集研究孕早期维生素D缺乏与早产结局相关性的队列研究和病例对照研究,检索时限均为建库至2019年6月.由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用RevMan 5.3软件进行Meta分析.结果 最终纳入6篇队列研究和3篇巢式病例对照研究.共纳入新生儿30 891例,其中早产儿1 912例;3篇中文文献,6篇英文文献;其中3篇研究来自中国,3篇来自北美,2篇来自欧洲,1篇来自澳大利亚.各文献中维生素D缺乏和早产的诊断标准基本一致.在调整年龄、种族等混杂因素后,Meta分析结果显示,孕早期维生素D缺乏不会增加早产的风险(OR=1.04,95% CI0.90~ 1.20,P=0.63).按照研究类型、测定方法、地区人群进行亚组分析,结果与总体结果一致.Meta分析结果未见明显发表偏倚.结论 当前证据表明,孕早期维生素D缺乏与早产很可能无相关性.“,”Objective To systematically evaluate the correlation between vitamin D deficiency in early pregnancy and the outcome of preterm birth.Method PubMed,Embase,the Cochrane Library,Web of Science,Ebsco,CBM,CNKI and Wanfang Data databases were searched to collect cohort studies and case-control studies on the correlation between vitamin D deficiency in early pregnancy and preterm birth outcomes,and the retrieval time was from the establishment of the database to June 2019.Two researchers independently reviewed the literature,extracted the data and evaluated the risk of bias in the included studies.RevMan 5.3 software was used for Meta analysis.Result A total of 6 cohort studies and 3 nested case-control studies were included.A total of 30 891 newborns were included,including 1 912 premature infants.3 Chinese articles and 6 English articles were reviewed including three studies from China,three from North America,two from Europe and one from Australia.The diagnostic criteria for vitamin D deficiency and preterm birth were similar in these studies.After adjusting for age,race and other confounding factors,Meta-analysis results showed that vitamin D deficiency in early pregnancy did not increase the risk of preterm birth (OR =1.04,95% CI 0.90 ~ 1.20,P =0.63).Subgroup analysis were conducted according to the study type,measurement method and regional population,and the results were consistent with the overall results.No significant publication bias was found in the meta-analysis results.Conclusion Current evidence suggests that vitamin D deficiency in early pregnancy has no significant influence on preterm birth.
