目的 探讨胸腺生发中心及细胞凋亡相关基因Bcl 2蛋白表达水平与获得性自身免疫性重症肌无力的关系。方法 将 31例重症肌无力患者的胸腺标本作为观察组 ,2 5例先心病胸腺标本作为正常对照组 ,分别行SP和HE染色 ,检测生发中心和Bcl 2的表达水平。结果 正常对照组无生发中心发现。而观察组Bcl 2表达水平明显高于对照组 ,且多为高表达者 ,两者对比差异极显著 (P <0 0 1)。结论 生发中心的出现和数目增多是胸腺增生的一个重要的形态学标志 ,Bcl 2高表达提示增生胸腺中免疫细胞的凋亡受到抑制 ,可能是获得性自身免疫性重症肌无力发生的重要机制之一。
Objective To investigate the relationship between the expression of Bcl-2 protein and the acquired autoimmune myasthenia gravis in the center of thymus germinal center and apoptosis. Methods Twenty-one thymus specimens of myasthenia gravis patients were selected as the observation group and twenty-five thymus specimens of the congenital heart disease as the normal control group. SP and HE staining were performed respectively to detect the expression of Bcl-2 and germinal center. Results The normal control group found no germinal centers. The expression of Bcl-2 in the observation group was significantly higher than that in the control group, and most of them were highly expressed, the difference between the two groups was significant (P <0.01). Conclusions The appearance and number of germinal centers are an important morphological marker of thymus hyperplasia. High expression of Bcl 2 suggests that apoptosis of immune cells in proliferative thymus may be inhibited and may be an important mechanism of acquired autoimmune myasthenia gravis one.
