目的观察马来酸桂哌奇特注射液对脑出血病人脑血流的影响。方法选择2009年2月~2010年2月在济南市第三人民医院诊治的脑出血患者60例,随机分为对照组和治疗组。对照组给予内科常规治疗,应用脱水剂、神经营养药物、脑细胞代谢活化剂、支持及对症治疗。治疗组在对照组治疗的基础上予马来酸桂哌奇特注射液160mg,加入生理盐水250ml,静脉滴注,每日1次,连用21天。两组病人分别在治疗后的第14天、第28天复查脑CT、患侧大脑中动脉脑血流图,观察血肿吸收情况、血肿量、血肿周围水肿面积、CT值、脑血流量的变化,依据临床表现和神经功能缺损积分判定临床疗效、有无血肿扩大及继发出血、有无并发症。结果治疗后第14天,治疗组病人的平均流速、阻力指数下降,脉动指数明显升高,与对照组相比有显著差异。治疗后第28天,两组病人的平均流速、脉动指数、阻力指数相比有显著性差异。结论马来酸桂哌奇特注射液治疗急性期脑出血,能防止脑血管痉挛,明显改善脑血流、脑循环,改善脑组织的缺血、缺氧状况,加快血肿吸收,有利于病人神经功能的恢复。
Objective To observe the effect of cinnamon maleate injection on cerebral blood flow in patients with intracerebral hemorrhage. Methods Sixty ICH patients diagnosed and treated in the Third People ’s Hospital of Jinan City from February 2009 to February 2010 were randomly divided into control group and treatment group. The control group was given routine medical treatment, the application of dehydration agents, neurotrophic drugs, brain cell metabolism activators, support and symptomatic treatment. Treatment group in the control group based on the treatment of cinnamic acid injection of maleic acid 160mg, added 250ml of saline, intravenous infusion, once daily for 21 days. The CT scan of cerebral CT and ipsilateral middle cerebral artery were observed on the 14th and 28th day after treatment in both groups, and the changes of hematoma absorption, hematoma volume, perihematomal edema area, CT value and cerebral blood flow were observed , Based on clinical manifestations and neurological deficit score to determine the clinical efficacy, with or without hematoma expansion and secondary bleeding, with or without complications. Results On the 14th day after treatment, the average flow velocity and the resistance index of the patients in the treatment group decreased and the pulsatility index increased significantly, which was significantly different from the control group. On the 28th day after treatment, the average flow velocity, pulsatility index and resistance index of two groups of patients were significantly different. Conclusion Cinnamic acid maleate injection in the treatment of acute cerebral hemorrhage can prevent cerebral vasospasm, significantly improve cerebral blood flow, cerebral circulation, improve brain ischemia, hypoxia, accelerate the absorption of hematoma, is conducive to the patient’s neurological function Recovery.
