目的应用胸阻抗法研究中老年高血压与高血压前期患者的血流动力学特点。方法 462例受检者分为高血压组、高血压前期组与正常组。应用LIFEGARD II型无创血流动力监测仪测定各组血流动力学指标:心搏量(SV)、体循环血管阻力(SVR)、胸液成分(TFC)与左心做功指数(LCWI)等。比较3组之间的血流动力学指标差异,并进一步分析高血压组患者血流动力学特点。结果高血压组与正常组之间比较,SBP、DBP、PP、MAP、SVR、SV、LCWI等指标存在统计学差异(P<0.05)。高血压前期组与正常组之间比较,SBP、PP、MAP、SVR存在统计学差异(P<0.05)。58.8%的高血压组患者存在SVR异常升高。结论高血压与高血压前期的患者中均存在不同程度的血流动力学异常。无创血流动力学监测可为高血压患者的个体化治疗提供参考。
Objective To study the hemodynamic characteristics of patients with middle-aged and elderly patients with hypertension and prehypertension by using chest impedance method. Methods 462 subjects were divided into hypertensive group, prehypertensive group and normal group. The hemodynamics indexes such as stroke volume (SV), systemic vascular resistance (SVR), pleural fluid composition (TFC) and left ventricular function index (LCWI) were determined by LIFEGARD II noninvasive blood flow monitor. The differences of hemodynamics between the three groups were compared, and the hemodynamic characteristics of hypertensive patients were further analyzed. Results There were significant differences in SBP, DBP, PP, MAP, SVR, SV, LCWI between hypertensive group and normal group (P <0.05). There were significant differences in SBP, PP, MAP and SVR between prehypertension group and normal group (P <0.05). 58.8% of hypertensive patients with abnormal SVR existence. Conclusions There are different degrees of hemodynamic abnormalities in both hypertension and prehypertensive patients. Noninvasive hemodynamic monitoring can provide a reference for the individualized treatment of patients with hypertension.
