目的明确我国公立医疗机构间职工流动的现状。方法通过分层整群抽样方法选取了全国各级各类376所样本公立医疗机构,获得流动职工个人人事方面和调动去向信息,分析东中西部地区人员的地区流向和流动人员素质的变化趋势。结果中西部地区公立医疗机构的人力都为净损失,5年半损失分别为9203人和679人,东部则净流入9882人;总体上,中东部公立医疗机构的人力素质在5年中得到明显改善,西部虽然从学历来看,人力素质也在提升,但结合职称和流出人员的素质因素分析后发现,总体上变化不明显,并且中西部与东部的人力素质相对差距有扩大的趋势。结论我国公立医疗机构的人员流动存在中西部向东部流动的现象,西部医疗机构卫生人力的素质处在徘徊状态,并且中部人力素质提高的程度不及东部地区,这势必将导致东部地区具有更大的竞争优势而加剧医疗资源地区不平衡的状况,从长远来看,无疑会对地区之间的卫生服务公平性造成不利影响。
Objective To clarify the status quo of the mobility of workers among public medical institutions in China. Methods A stratified cluster sampling method was used to select 376 public medical institutions of various kinds at all levels in China, obtaining personal and professional information of migrant workers and the motivation of migrant workers, and analyzing the trends of regional mobility and quality of migrant workers in eastern, central and western regions.
