目的:探讨扩散加权成像(DWI)中不同扩散敏感系数下ADC值及相对ADC值对乳腺良恶性病变鉴别诊断价值。方法对41例经病理结果证实的乳腺病变行DWI,b值分别为800、1000 s/mm2,测量病变的ADC值及对侧正常乳腺的ADC值,同时计算病变平均 ADC值及相对 ADC值。结果两组 b值中恶性病变平均ADC值及相对ADC值均低于良性病变,乳腺良恶性病变的ADC值均随b值增大而降低,但是相对ADC值无明显差异。采用非参数法受试者工作特性曲线统计学方法得到各诊断指标ROC曲线下面积,并确定其鉴别良恶性病变诊断阈值。结论 b=800s/mm2诊断价值优于 b=1000s/mm2,ADC值诊断价值较高,相对ADC值在不同b值间较接近,无显著差异。“,”Objective To evaluate the values of apparent dif usion coef icient(ADC) and relative ADC of dif usion-weighted imaging (DWI) in distinguishing malignant from benign breast lesions. Methods 41 patients with pathological y confirmed breast lesions underwent DWI, b values were 800,1000 s/mm2.The ADC values of the lesions and contralateral normal breast were measured, also calculated the average ADC values and relative ADC (rADC)values of lesions. Results The average ADC values and rADC values of malignant lesions were lower than that of the benign lesion.The ADC values of lesions were decreased with increased b value,but there was no significant dif erence in rADC values. The areeas under ROC cures were calculated by Non-parametric ROC cure Statistical Methods The thresholds which could balance the needs of sensity and specificity were also determined. Conclusion b=800s/mm2 is breast best b value of DWI, ADC values were of high diagnostic value, rADC values between the dif erent b values have no significant dif erence.
