本文通过硬膜外麻醉选择性剖腹产时,应用多普勒技术评价子宫胎盘及脐循环与母体股动静脉血流速的变化相互关系。观察无合并症单胎妊娠13例(初产妇8例、经产妇5例),选择性剖腹产指征为臀先露8例,头盆不称5例。多普勒脉冲技术同过去的报道,在子宫的胎盘侧检查子宫胎盘循环,记录4~6连续心搏周期,用Doptek光谱分析仪分析8~10个胎儿脐环的阻力指数及脉冲指数。用纵波扫描M-型技术计算股动脉的峰流速。测量股动静脉直径。测定血压及心率,并经脐多普勒测胎儿心率(FHR)。所有患者皆取左倾斜位右髋后加垫行多普勒测定,于侧卧位在L_(3~4)作硬膜外麻醉,注入含1/20万肾上腺素2%利多卡因3ml。15分钟后仰卧半斜位作第二次多普勒血流测量。
In this paper, epidural anesthesia selective caesarean section, the application of Doppler ultrasound evaluation uterine placenta and umbilical circulation and maternal femoral venous blood flow changes in the relationship. No complications were observed in 13 singleton pregnancies (8 cases of primipara, 5 cases of mothers), selective caesarean section indications were 8 cases of gluteal revelation, 5 cases of cephalopelvic disproportion. The resistance and pulse indices of 8 to 10 fetal umbilical rings were analyzed using a Doptek spectrometer. Compute the peak flow velocity of femoral artery by longitudinal wave scanning M-type technique. Measurement of femoral vein diameter. Blood pressure and heart rate were measured and fetus heart rate (FHR) was measured by umbilical Doppler. All patients took the right hip of the left oblique after pad line Doppler measurements in the lateral position of the L_ (3-4) for epidural anesthesia, injection of 1/2 million 2% lidocaine containing 3ml. Fifteen minutes after supine hemi-oblique for the second Doppler flow measurement.
